Description
Overview of the medicine
Recogen 5000 IU Injection is a form of Human Chorionic Gonadotropin (HCG), a hormone that supports the normal development of an egg in a woman's ovary, and stimulates its release during ovulation. In men, it is used to increase sperm count and treat undescended testicles.
Uses & Indications
Dosage
Adults
Female Infertility: 5000-10000 IU administered one day after the last dose of menotropins or FSH. Male Hypogonadism: 500-4000 IU three times weekly for several weeks or months. Cryptorchidism: 1000-5000 IU three times weekly for 4-6 weeks.
Elderly
No specific dose adjustment is generally required, but caution should be exercised in elderly patients with pre-existing cardiovascular disease.
Renal_impairment
No specific dosage adjustments are typically required, but monitoring is advised.
How to Take
Recogen 5000 IU Injection is administered by intramuscular (IM) or subcutaneous (SC) injection after reconstitution with the provided solvent. The injection site should be rotated to prevent lipoatrophy.
Mechanism of Action
HCG is an LH (Luteinizing Hormone) analog. It binds to LH receptors in the gonads, stimulating steroidogenesis. In women, it induces ovulation and maintains the corpus luteum. In men, it stimulates Leydig cells to produce testosterone and promotes spermatogenesis.
Pharmacokinetics
Onset
Ovulation typically occurs within 36-40 hours after HCG administration in women.
Excretion
Mainly excreted by the kidneys, with approximately 10-12% excreted unchanged in urine.
Half life
Biphasic elimination. Initial half-life is approximately 11 hours, followed by a slower phase of approximately 2-3 days.
Absorption
Rapidly and well absorbed following intramuscular (IM) or subcutaneous (SC) administration. Peak plasma concentrations are reached within 6-16 hours for IM and 6-16 hours for SC.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to HCG or any excipients.
- Uncontrolled thyroid or adrenal dysfunction.
- Primary gonadal failure (e.g., ovarian failure, testicular agenesis).
- Presence of androgen-dependent tumors (e.g., prostate carcinoma, breast carcinoma in males).
- Unexplained vaginal bleeding.
- Pregnancy or lactation (when used for fertility treatment).
Drug Interactions
Other fertility drugs
May be used with other fertility medications (e.g., FSH, menotropins), but close monitoring is essential.
GnRH agonists/antagonists
Used in conjunction with GnRH analogs, careful monitoring of ovarian response is required.
Storage
Store in a refrigerator at 2°C to 8°C. Protect from light. Do not freeze. After reconstitution, the solution should be used immediately. If not, it can be stored for up to 24 hours at 2°C to 8°C.
Overdose
Overdose in women may lead to Ovarian Hyperstimulation Syndrome (OHSS), characterized by severe abdominal pain, bloating, nausea, vomiting, and weight gain. In men, excessive androgenic effects or gynecomastia may occur. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category X: HCG is used to achieve pregnancy, but is contraindicated once pregnancy is established. Not recommended for use during lactation due to potential passage into breast milk and effects on the infant.
