Refain 50 mg Tablet contains Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID), used for the relief of pain and inflammation in various musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Typically, 50 mg twice daily, or as directed by the physician. The usual dose for aceclofenac is 100 mg twice daily, so 50mg might be used for dose titration or specific conditions.
Elderly
Lower doses may be considered for elderly patients, or as advised by a physician.
Renal_impairment
Caution required; dosage adjustment may be necessary in patients with mild to moderate renal impairment. Avoid in severe renal impairment.
How to Take
Administer orally. The tablet should be swallowed whole with sufficient liquid, preferably with or after food to minimize gastrointestinal discomfort.
Mechanism of Action
Aceclofenac inhibits prostaglandin synthesis by inhibiting the cyclooxygenase (COX) enzyme, primarily COX-2, reducing inflammation, pain, and fever.

Onset
Pain relief usually starts within 30-60 minutes.
Excretion
Approximately two-thirds of the dose is excreted in the urine, mainly as conjugated metabolites, and one-third in the faeces.
Half life
Approximately 4-4.3 hours.
Absorption
Rapidly and completely absorbed after oral administration, peak plasma concentrations reached in 1.25 to 3 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes to 4'-hydroxyaceclofenac and other metabolites.

Contraindications
- Hypersensitivity to aceclofenac or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions

Increased risk of gastrointestinal side effects.
Lithium, Digoxin, Methotrexate
Increased plasma levels and toxicity.
Warfarin, other anticoagulants
Increased risk of bleeding.

Reduced antihypertensive effect and increased risk of renal impairment.

Overdose
Symptoms may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, occasionally convulsions. Management is primarily supportive and symptomatic, including gastric lavage or activated charcoal if recent ingestion.
Pregnancy & Lactation
Not recommended in the third trimester of pregnancy due to potential fetal harm. Use with caution in the first two trimesters only if clearly needed and benefits outweigh risks. Not recommended during breastfeeding as aceclofenac is excreted in small amounts in breast milk.
