Overview of the medicine
Relacef 250 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the urinary tract, respiratory tract, ears, nose, throat, and gonorrhea.
Uses & Indications
Dosage
Adults
The usual adult dose is 200-400 mg once daily or 200 mg every 12 hours. For 250 mg tablet, typically 250 mg twice daily.
Elderly
Similar to adult dosage; however, renal function should be assessed, and dosage adjusted if necessary.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance less than 60 mL/min. For CrCl 21-60 mL/min, 75% of the standard dose; for CrCl <20 mL/min, 50% of the standard dose.
How to Take
Relacef 250 mg Tablet should be taken orally, with or without food. It is recommended to take the tablet with a full glass of water.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis, leading to the lysis and death of the bacterial cells. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile.
Half life
Plasma half-life is typically 3-4 hours, which can be prolonged in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food can slightly delay absorption but does not significantly alter the extent of absorption.
Metabolism
Cefixime is not extensively metabolized in the body. No active metabolites have been identified.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •Known hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
Concurrent use may increase cefixime's bioavailability.
Carbamazepine
Increased carbamazepine levels have been reported when co-administered with cefixime.
Oral Contraceptives
Some cephalosporins may reduce the effectiveness of oral contraceptives, though this is rare with cefixime.
Warfarin and Anticoagulants
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing bleeding risk. Monitor coagulation parameters.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing mother.
