Description
Overview of the medicine
Reladol 100 mg Injection contains Tramadol Hydrochloride, a centrally acting synthetic opioid analgesic. It is used for the management of moderate to moderately severe pain.
Uses & Indications
Dosage
Adults
100 mg (2 mL) intramuscularly or intravenously slowly, repeated every 4-6 hours as needed, not exceeding 400 mg/day.
Elderly
Initial doses should be carefully titrated; generally, the usual adult dose may be used, but lower initial doses and longer dosing intervals may be considered for patients over 75 years.
Renal_impairment
For creatinine clearance < 30 mL/min, extend the dosing interval to 12 hours. Not recommended for patients on dialysis.
How to Take
Administer slowly by intravenous injection (over 2-3 minutes) or as an intravenous infusion, or intramuscularly. Do not administer intrathecally or epidurally.
Mechanism of Action
Tramadol acts as an opioid agonist at mu-opioid receptors. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effects.
Pharmacokinetics
Onset
Within 15-30 minutes after IV administration.
Excretion
Mainly excreted renally (approximately 90%) as metabolites and unchanged drug; small amounts are excreted in feces.
Half life
Approximately 5-7 hours for tramadol and 7-9 hours for its active metabolite O-desmethyltramadol.
Absorption
Rapidly and almost completely absorbed after parenteral administration. Peak plasma concentration reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by N- and O-demethylation, and by glucuronidation. O-desmethyltramadol (M1) is the primary active metabolite.
Side Effects
Contraindications
- Hypersensitivity to tramadol or other opioids
- Acute intoxication with alcohol, hypnotics, analgesics, opioids, or other psychotropic drugs
- Patients receiving MAO inhibitors or within 14 days of stopping them
- Epilepsy not controlled by treatment
- Severe respiratory depression
Drug Interactions
Carbamazepine
Significant reduction in tramadol analgesic effect due to increased metabolism.
MAO Inhibitors
Risk of serotonin syndrome and seizures.
Serotonergic Drugs (e.g., SSRIs, SNRIs)
Increased risk of serotonin syndrome.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased risk of respiratory depression, sedation, coma, and death.

Symptoms include respiratory depression, somnolence progressing to coma, seizures, skeletal muscle flaccidity, cold and clammy skin, and bradycardia. Treatment involves supportive care, naloxone for respiratory depression, and benzodiazepines for seizures.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the third trimester due to risk of neonatal withdrawal syndrome. Excreted in breast milk; caution advised or alternative pain relief considered.
