Description
Overview of the medicine
Relaxium 3 mg Tablet is a medication primarily used for the short-term management of anxiety symptoms, insomnia, and muscle spasms. It acts on the central nervous system to produce a calming effect.
Uses & Indications
Dosage
Adults
For anxiety: 1 mg two to three times daily, or 3 mg once daily at bedtime. Dosage may be adjusted based on response, up to a maximum of 9 mg/day. For insomnia: 3 mg at bedtime.
Elderly
Initial dose of 0.5-1 mg once daily, preferably at bedtime, or 1-1.5 mg once daily, adjusted cautiously to minimize adverse effects.
Renal_impairment
No specific dose adjustment is required for mild to moderate renal impairment, but caution is advised in severe cases. Initiate with lower doses.
How to Take
Administer orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush, chew, or break the tablet.
Mechanism of Action
Relaxium enhances the effect of gamma-aminobutyric acid (GABA), a major inhibitory neurotransmitter in the brain, by binding to specific GABA-A receptors. This increases chloride ion influx, leading to hyperpolarization and stabilization of neuronal membranes, resulting in reduced neuronal excitability and a calming effect.
Pharmacokinetics
Onset
Onset of action typically occurs within 30-60 minutes.
Excretion
Mainly excreted in the urine as inactive metabolites.
Half life
Elimination half-life ranges from 10 to 20 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are typically reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (e.g., CYP3A4) into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Relaxium or any excipients
- •Acute narrow-angle glaucoma
- •Severe respiratory insufficiency
- •Sleep apnea syndrome
- •Severe hepatic impairment
- •Myasthenia gravis
Drug Interactions
Alcohol
Increased CNS depressant effects, leading to severe sedation, respiratory depression, and impaired psychomotor function.
Opioids
Significantly increased risk of profound sedation, respiratory depression, coma, and death.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin, ritonavir)
May increase plasma concentrations of Relaxium, potentially enhancing its effects and side effects.
CYP3A4 Inducers (e.g., rifampin, carbamazepine, St. John's Wort)
May decrease plasma concentrations of Relaxium, potentially reducing its efficacy.
Other CNS Depressants (e.g., antidepressants, antipsychotics, antihistamines)
Additive CNS depressant effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include somnolence, confusion, diminished reflexes, impaired coordination, and coma. In severe cases, respiratory depression and hypotension may occur. Management involves general supportive measures, maintaining a patent airway, gastric lavage, and intravenous fluids. Flumazenil, a benzodiazepine receptor antagonist, may be considered in severe cases, but caution is advised due to the risk of inducing seizures.
Pregnancy & Lactation
Pregnancy Category D. Relaxium should not be used during pregnancy, especially during the first trimester, due to potential for fetal harm, including congenital malformations and withdrawal symptoms in neonates. It is excreted in breast milk; therefore, use during lactation is contraindicated due to potential for sedation, poor feeding, and weight loss in infants.
