Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of seasonal allergies. The 2 mg chewable tablet is typically prescribed for pediatric patients (2-5 years of age).
Uses & Indications
Dosage
Adults
For children 2-5 years: One 2 mg chewable tablet orally once daily in the evening. For adults and adolescents (15 years and older), a 10 mg tablet is typically prescribed.
Elderly
For children 2-5 years: One 2 mg chewable tablet orally once daily in the evening. For elderly patients (15 years and older), a 10 mg tablet is typically prescribed.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take one tablet orally once daily in the evening, with or without food. Chew the tablet completely before swallowing.
Mechanism of Action
Montelukast selectively binds to and inhibits the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are inflammatory mediators that cause bronchoconstriction, mucus secretion, and increased vascular permeability, contributing to asthma and allergic rhinitis symptoms. By blocking CysLT1 receptors, montelukast prevents these effects.

Onset
Improvement in symptoms typically seen within one day.
Excretion
Primarily excreted in the feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in 2-4 hours. Bioavailability is about 64%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4, CYP2C9, and CYP2C8).

Drug Interactions
Rifampicin
May decrease montelukast plasma concentrations.
Phenobarbital
May decrease montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, supportive and symptomatic treatment is recommended. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Montelukast is excreted in breast milk; use with caution in nursing mothers.
