Description
Overview of the medicine
Relmas (Relugolix) is a non-peptide gonadotropin-releasing hormone (GnRH) receptor antagonist used for the treatment of uterine fibroids, endometriosis, and advanced prostate cancer. It works by inhibiting the release of gonadotropins, thereby reducing estrogen and testosterone levels.
Uses & Indications
Dosage
Adults
For uterine fibroids/endometriosis: typically 40 mg once daily (often in combination). For prostate cancer: 120 mg once daily.
Elderly
No specific dose adjustment required based on age alone. Monitor for age-related renal/hepatic impairment.
Renal_impairment
Mild to moderate renal impairment: No dose adjustment. Severe renal impairment: Consider dose reduction.
How to Take
Take orally once daily with or without food. Swallow the tablet whole.
Mechanism of Action
Relugolix selectively binds to and antagonizes the GnRH receptors in the anterior pituitary gland, which reduces the release of luteinizing hormone (LH) and follicle-stimulating hormone (FSH). This suppression of gonadotropins leads to a decrease in ovarian estrogen and progesterone production in females and testicular testosterone production in males.
Pharmacokinetics
Onset
Hormone suppression typically starts within days to weeks.
Excretion
Mainly excreted via feces (approximately 81%), with a smaller portion excreted via urine (approximately 19%).
Half life
Approximately 25-60 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized by CYP3A4, with minor contributions from other CYP enzymes.
Side Effects
Contraindications
- Known hypersensitivity to Relugolix or any components
- Undiagnosed abnormal uterine bleeding
- Pregnancy
- Severe hepatic impairment
Drug Interactions
P-gp inhibitors
May increase Relugolix concentrations. Co-administration generally not recommended or requires dose adjustment.
Strong CYP3A inducers
May decrease Relugolix concentrations, reducing efficacy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. In case of suspected overdose, provide supportive care and monitor for adverse effects.
Pregnancy & Lactation
Contraindicated in pregnancy due to potential harm to the fetus. It is not known if Relugolix is excreted in human milk; breastfeeding is not recommended.
