Description
Overview of the medicine
Relnac Plus 75 mg Injection is a combination medication containing Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID), and B Vitamins (Thiamine, Pyridoxine, Cyanocobalamin). It is used to relieve pain, inflammation, and neuropathic symptoms often associated with musculoskeletal disorders, nerve pain, and other inflammatory conditions. The B vitamins support nerve health and metabolism.
Uses & Indications
Dosage
Adults
Usually 1 ampoule (75 mg Diclofenac + B vitamins) daily, given by deep intramuscular injection into the gluteal muscle. In severe cases, up to 2 ampoules per day, given at different sites. Not recommended for more than 2-3 days without switching to oral therapy.
Elderly
Use with caution, lowest effective dose, monitor renal function.
Renal_impairment
Use with caution, consider dose reduction in severe impairment.
How to Take
Deep intramuscular injection into the gluteal muscle. Ensure correct injection technique to avoid nerve damage. Do not administer intravenously.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. Thiamine, Pyridoxine, and Cyanocobalamin act as coenzymes in various metabolic processes, particularly important for nerve function and myelin sheath formation, and contribute to nerve regeneration and pain modulation.
Pharmacokinetics
Onset
Diclofenac: 15-30 minutes for pain relief.
Excretion
Diclofenac: Primarily renal (60%) and biliary/fecal (35%). B vitamins: Primarily renal, excess excreted unchanged.
Half life
Diclofenac: 1-2 hours. B vitamins: Vary (e.g., Pyridoxine ~15-20 days for active form, Cyanocobalamin days to months).
Absorption
Diclofenac: Rapidly and completely absorbed after IM administration. B vitamins: Readily absorbed.
Metabolism
Diclofenac: Primarily hepatic metabolism via CYP2C9 and CYP2C8, forming hydroxylated and glucuronidated metabolites. B vitamins: Metabolic conversion to active coenzyme forms.
Side Effects
Contraindications
- •Hypersensitivity to Diclofenac, aspirin, or other NSAIDs, or any B vitamins
- •Active gastrointestinal bleeding or ulceration
- •Severe renal or hepatic impairment
- •Severe heart failure
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Last trimester of pregnancy
Drug Interactions
Cyclosporine
Increased nephrotoxicity.
Other NSAIDs
Increased risk of side effects.
Corticosteroids
Increased risk of gastrointestinal ulceration.
Antidiabetic agents
Altered blood glucose levels (rare).
Diuretics and ACE inhibitors
Reduced antihypertensive effect, increased risk of renal impairment.
Lithium, Digoxin, Methotrexate
Increased plasma levels of these drugs.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C, protected from light and moisture. Do not freeze.
Overdose
Symptoms: Lethargy, drowsiness, nausea, vomiting, epigastric pain, GI bleeding, rare acute renal failure, hepatic damage. Management: Symptomatic and supportive treatment. No specific antidote. Gastric lavage and activated charcoal may be considered if recent oral ingestion.
Pregnancy & Lactation
Pregnancy: Contraindicated in the last trimester due to potential premature closure of the fetal ductus arteriosus. Use in first two trimesters only if clearly necessary and potential benefits outweigh risks. Lactation: Diclofenac and its metabolites pass into breast milk in small amounts; use with caution. B vitamins are generally safe.
