Description
Overview of the medicine
Remacid 175 mg Suspension contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against various Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Typically 250 mg twice daily for most infections. For severe infections or certain conditions, dosage may be increased up to 500 mg twice daily. For uncomplicated gonorrhea, a single 1 gm dose.
Elderly
No specific dose adjustment is usually required unless severe renal impairment is present.
Renal_impairment
Dosage reduction may be necessary. For creatinine clearance 10-29 mL/min, 250 mg once daily. For <10 mL/min, 250 mg every 48 hours.
How to Take
Oral administration. Take with food to enhance absorption and reduce gastrointestinal upset. Shake the suspension well before use.
Mechanism of Action
Cefuroxime Axetil works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to cell lysis and bacterial death.

Excretion
Excreted unchanged in the urine, mainly via glomerular filtration and tubular secretion.
Half life
Approximately 1 to 1.5 hours.
Absorption
Well absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime in the intestinal mucosa and blood. Absorption is enhanced by food.
Metabolism
Not extensively metabolized; primarily hydrolyzed to the active compound cefuroxime.
Side Effects
Contraindications
- Hypersensitivity to Cefuroxime, other cephalosporins, or any component of the formulation
- History of severe hypersensitivity reactions to any other type of beta-lactam antibacterial agent (e.g., penicillins)
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the plasma concentration versus time curve by about 50%, prolonging cefuroxime's activity.
Loop diuretics
Concurrent administration with potent loop diuretics (e.g., furosemide) may enhance nephrotoxicity, especially in patients with pre-existing renal impairment.
Oral contraceptives
May reduce the efficacy of oral contraceptives. Additional non-hormonal contraception is recommended.
Antacids/H2 blockers/Proton pump inhibitors
May reduce the bioavailability of cefuroxime axetil, especially due to a reduction in gastric acidity. Should be taken at least 1-2 hours apart.
Storage
Store dry powder below 30°C in a cool, dry place, away from light and moisture. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 10 days.
Overdose
Overdose symptoms may include cerebral irritation leading to convulsions. Management is primarily supportive. Hemodialysis and peritoneal dialysis can remove cefuroxime from the body.

Pregnancy Category B. Cefuroxime crosses the placenta. Use in pregnancy only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but caution is advised.
