Description
Overview of the medicine
Remaprost is a prostaglandin analog used to reduce elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) once daily in the evening. Do not exceed one dose per day as more frequent administration may decrease the IOP-lowering effect.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
For ophthalmic use only. Instill one drop into the conjunctival sac of the affected eye(s) once daily in the evening. If using other ophthalmic medications, allow at least 5 minutes between applications. Remove contact lenses prior to use and reinsert 15 minutes after administration.
Mechanism of Action
Remaprost reduces intraocular pressure by increasing the outflow of aqueous humor primarily through the uveoscleral pathway, and possibly by increasing trabecular outflow.
Pharmacokinetics
Onset
Onset of IOP reduction typically within 3-4 hours after administration.
Excretion
Excreted mainly by the kidneys as inactive metabolites.
Half life
The active acid of Remaprost has a short plasma half-life (e.g., approximately 17 minutes).
Absorption
Well absorbed through the cornea where it is hydrolyzed to its biologically active acid. Systemic absorption is minimal.
Metabolism
Primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Remaprost or any other component of the eye drop.
- Active intraocular inflammation (e.g., iritis/uveitis).
Drug Interactions
Other Prostaglandin Analogs
Concomitant use with other prostaglandin analogs (e.g., bimatoprost, travoprost) is not recommended as it may lead to paradoxical increases in IOP. However, concomitant use of two or more prostaglandin analogs is generally discouraged.
Thimerosal-containing products
May lead to precipitation when used with thimerosal-containing products.
Storage
Store at room temperature (15°C to 25°C), protected from light. Do not freeze.
Overdose
Overdose is unlikely with topical ocular administration. If overdose occurs, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Use in pregnant women only if the potential benefit justifies the potential risk to the fetus. It is not known whether Remaprost is excreted in human milk; use with caution in breastfeeding women.

Clinical Trials
Extensive Phase III clinical trials demonstrated the efficacy and safety of Remaprost in reducing intraocular pressure in patients with open-angle glaucoma and ocular hypertension. Key studies focused on sustained IOP reduction over 12 months.
Lab Monitoring
- Regular monitoring of intraocular pressure (IOP) is essential.
- Routine laboratory tests for systemic effects are generally not required.
Doctor Notes
- Emphasize single daily dosing in the evening.
- Counsel patients on potential cosmetic side effects such as iris color change and eyelash growth.
- Monitor IOP regularly and evaluate for signs of ocular inflammation or macular edema.
Patient Guidelines
- Follow proper administration technique to avoid contamination of the dropper tip and solution.
- Inform your doctor if you experience any new eye symptoms, eye trauma, or eye surgery.
- Remove contact lenses before applying Remaprost and wait at least 15 minutes before reinserting.
Missed Dose Advice
If a dose is missed, continue treatment with the next dose as scheduled. Do not administer a double dose to make up for a missed dose.
Driving Precautions
Remaprost may cause transient blurred vision upon instillation. Patients should be advised to exercise caution when driving or operating machinery until their vision clears.
Lifestyle Advice
- Regularly attend eye check-ups as advised by your ophthalmologist.
- Avoid rubbing your eyes after applying the drops to prevent irritation.
