Description
Overview of the medicine
Remathrin 5% w/w Cream is a topical medication containing permethrin, used for the treatment of scabies caused by the mite Sarcoptes scabiei. It is also used to treat pediculosis (head lice, body lice, and pubic lice).
Uses & Indications
Dosage
Adults
For scabies: Apply a thin layer of cream from the head to the soles of the feet, rub in thoroughly. Leave on for 8-14 hours, then wash off. One application is usually sufficient, but a second application may be needed after 7-14 days if living mites are present. For lice: Apply to affected area, leave on for 10 minutes, then rinse.
Elderly
Same as adult dosage. For scabies, apply from the head to the soles of the feet. Ensure all skin folds are covered.
Renal_impairment
No dosage adjustment needed for topical application as systemic absorption is minimal.
How to Take
For external use only. Apply to clean, dry skin. Do not ingest. Avoid contact with eyes and mucous membranes (mouth, nose, vagina, anal area). If contact occurs, rinse thoroughly with water. Hands should be washed after application.
Mechanism of Action
Permethrin acts on the neuron membranes of arthropods (mites and lice), causing depolarization by disrupting the sodium channel current that regulates the polarization of the membrane. This leads to paralysis and death of the parasites.

Onset
Typically effective within hours after application, though pruritus (itching) may persist for up to several weeks post-treatment due to allergic reactions to dead mites.
Excretion
Excreted primarily in the urine as inactive metabolites.
Half life
Approximately 12-14 hours (for the systemically absorbed portion), undergoing rapid hydrolysis.
Absorption
Minimal systemic absorption (less than 2%) occurs after topical application of permethrin 5% cream.
Metabolism
Rapidly hydrolyzed by esterases to inactive metabolites in the liver and skin.
Side Effects
Contraindications
- •Hypersensitivity to permethrin, other pyrethroids or pyrethrins, or any component of the cream.
Drug Interactions
Corticosteroids (topical)
Concurrent use of topical corticosteroids may exacerbate some skin conditions masking the underlying scabies infestation.
Storage
Store below 30°C (86°F). Do not freeze. Keep away from direct heat and sunlight. Keep out of reach of children.
Overdose
Over-application or prolonged exposure may increase local side effects such as burning, itching, or erythema. Systemic overdose is unlikely due to minimal absorption. If ingested, supportive measures are advised, including gastric lavage if necessary.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown fetal harm, but adequate and well-controlled studies in pregnant women are lacking. Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Exercise caution when administering to a nursing mother; permethrin and/or its metabolites may be excreted in human milk. Temporary cessation of breastfeeding may be considered.
