Remicov 5 mg Injection contains Remdesivir, an antiviral medication used for the treatment of COVID-19 in adults and pediatric patients requiring hospitalization. It is administered intravenously.
Uses & Indications
Dosage
Adults
Loading dose: 200 mg on Day 1, followed by 100 mg once daily from Day 2 for 4-9 days, for a total treatment duration of 5-10 days depending on clinical response.
Elderly
No specific dose adjustment required based on age.
Renal_impairment
Not recommended for patients with eGFR <30 mL/min due to potential accumulation of excipient sulfobutylether beta-cyclodextrin (SBECD).
How to Take
For intravenous infusion only. Must be reconstituted and diluted prior to administration. Administer over 30-120 minutes.
Mechanism of Action
Remdesivir is a prodrug that is metabolized intracellularly to its active nucleoside triphosphate metabolite. This metabolite acts as an adenosine triphosphate (ATP) analog, interfering with the RNA-dependent RNA polymerase (RdRp) of SARS-CoV-2, leading to termination of viral RNA transcription and inhibition of viral replication.

Excretion
Mainly renal excretion of the active metabolite and other inactive metabolites. Small portion via feces.
Half life
Remdesivir: ~1 hour (parent drug). Active metabolite (GS-441524): ~20-25 hours (intracellular half-life).

Administered intravenously, thus 100% bioavailability. Rapidly converted to its active metabolite.
Metabolism
Primarily metabolized to the active nucleoside triphosphate. Hydrolysis by esterases and phosphorylation by host kinases.
Side Effects
Contraindications

- •Patients with severe renal impairment (eGFR <30 mL/min) due to excipient accumulation risk.
Drug Interactions
Chloroquine/Hydroxychloroquine
Coadministration is not recommended as they may antagonize the intracellular metabolic activation and antiviral activity of remdesivir.
Storage
Store unopened vials below 30°C (86°F). Do not freeze. Protect from light. Reconstituted solution: Store at 2°C to 25°C for up to 24 hours.
Overdose
No specific antidote. Management involves supportive care, including monitoring of vital signs and clinical status. Hemodialysis is not effective at removing significant amounts of remdesivir.
