Description
Overview of the medicine
Remivir 5 mg Injection is an antiviral medication used to treat certain severe viral infections. It works by inhibiting viral replication, thereby reducing the viral load in the body.
Uses & Indications
Dosage
Adults
5 mg/kg IV once daily for 7-10 days, infused over 30-60 minutes.
Elderly
No specific dose adjustment needed unless renal function is impaired.
Renal_impairment
Reduce dose to 2.5 mg/kg IV once daily for CrCl 30-50 mL/min; contraindicated if CrCl <30 mL/min.
How to Take
Administer intravenously after reconstitution. The solution should be clear and colorless. Do not administer as a rapid bolus injection.
Mechanism of Action
Remivir is a prodrug that is intracellularly phosphorylated to its active triphosphate form. This active metabolite competes with natural nucleoside triphosphates for incorporation into viral DNA/RNA by viral reverse transcriptase, leading to chain termination and inhibition of viral replication.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Primarily excreted renally (approximately 70-80% as unchanged drug and metabolites)
Half life
Approximately 3-5 hours (active metabolite 18-24 hours)
Absorption
Rapidly absorbed after intravenous administration; bioavailability is 100%. Peak plasma concentrations achieved within 1 hour.
Metabolism
Primarily metabolized intracellularly by kinases to its active triphosphate form. Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Remivir or any component of the formulation
- Severe renal impairment (CrCl <30 mL/min)
Drug Interactions
NSAIDs
May increase Remivir plasma levels and nephrotoxicity.
Amphotericin B
Increased risk of nephrotoxicity.
Storage
Store unreconstituted powder at 2°C to 8°C (36°F to 46°F). Protect from light. Reconstituted solution stable for 24 hours at room temperature.
Overdose
Symptoms include severe nephrotoxicity, hepatotoxicity, and bone marrow suppression. Treatment is supportive; hemodialysis may be considered for severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown if Remivir is excreted in human milk; use with caution in breastfeeding mothers.
