Remox (Amoxicillin) is a penicillin-type antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Standard dosage for mild to moderate infections: 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours. Higher doses for severe infections.
Elderly
Dosage adjustment may be necessary based on renal function due to age-related decline in kidney function.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), dosage reduction and/or extended dosing interval is required (e.g., 250 mg every 12 hours).
How to Take
Take orally with or without food. To maximize absorption and minimize gastrointestinal upset, some prefer to take it with food. Complete the full prescribed course of treatment.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 1-2 hours of oral administration.
Excretion
Primarily renal excretion (60-80% as unchanged drug) within 6-8 hours. Also eliminated in bile to a lesser extent.
Half life
Approximately 1-1.5 hours in adults with normal renal function; prolonged in renal impairment.
Absorption
Rapidly and well absorbed orally, bioavailability is approximately 70-90%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
About 10-25% is metabolized to inactive penicilloic acid; the remainder is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to penicillin antibiotics or any component of the formulation.
- •History of a severe hypersensitivity reaction (e.g., anaphylaxis) to any other beta-lactam agent (e.g., cephalosporins, carbapenems).
- •Infectious mononucleosis (due to increased risk of rash).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. INR monitoring recommended.
Probenecid
Concomitant use with probenecid prolongs the half-life and increases the plasma concentration of amoxicillin by competing for renal tubular secretion.
Methotrexate
Amoxicillin may reduce the renal excretion of methotrexate, potentially increasing methotrexate toxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives in some patients, leading to breakthrough bleeding or unintended pregnancy.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea), electrolyte imbalance, and crystalluria (renal failure may occur in severe cases). In patients with impaired renal function or those receiving very high doses, neurological effects including convulsions may occur. Treatment is largely symptomatic and supportive. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Generally considered safe for use during pregnancy when clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor the infant for diarrhea, candidiasis, or allergic sensitization.
