Description
Overview of the medicine
Renahaler is a combination inhaler used for the long-term maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It contains fluticasone furoate, an inhaled corticosteroid, and vilanterol, a long-acting beta2-adrenergic agonist.
Uses & Indications
Dosage
Adults
One inhalation (100 mcg Fluticasone Furoate / 25 mcg Vilanterol) once daily, at the same time each day.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment is required.
How to Take
For oral inhalation only. Do not use for acute asthma attacks or COPD exacerbations. Rinse mouth with water after each use to reduce the risk of oral candidiasis.
Mechanism of Action
Fluticasone Furoate works by reducing inflammation in the airways. Vilanterol works by relaxing the smooth muscles in the airways, leading to bronchodilation. The combination improves lung function and reduces symptoms.
Pharmacokinetics
Onset
Vilanterol: Within minutes (bronchodilation).
Excretion
Primarily excreted via feces, with minor urinary excretion.
Half life
Fluticasone Furoate: ~24 hours. Vilanterol: ~11 hours.
Absorption
Fluticasone Furoate: Systemic exposure is low due to high first-pass metabolism. Vilanterol: Rapidly absorbed, low absolute bioavailability.
Metabolism
Primarily metabolized by CYP3A4.
Side Effects
Contraindications
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- Hypersensitivity to fluticasone furoate, vilanterol, or any excipients.
Drug Interactions
0
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir): May increase systemic exposure to fluticasone furoate and vilanterol, leading to increased risk of side effects.
1
Beta-adrenergic Blockers: May antagonize the effect of vilanterol. Avoid non-cardioselective beta-blockers.
Storage
Store below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose may include tachycardia, tremors, headache, and hypokalemia. Management is supportive, and patients should be monitored for cardiac effects.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if components are excreted in human milk; use with caution during lactation.
