Description
Overview of the medicine
Renexin-XR 500 mg tablet contains Rifaximin, a non-systemic antibiotic primarily used to treat irritable bowel syndrome with diarrhea (IBS-D), traveler's diarrhea, and to reduce the risk of overt hepatic encephalopathy (HE) recurrence. Its action is largely confined to the gastrointestinal tract due to minimal systemic absorption.
Uses & Indications
Dosage
Adults
For Hepatic Encephalopathy or IBS-D: 500 mg twice daily as directed by physician. For Traveler's Diarrhea, typically 200 mg three times daily (if available in this strength, otherwise consult physician).
Elderly
No dosage adjustment generally required based on age alone.
Renal_impairment
No dosage adjustment required as systemic absorption is minimal.
How to Take
Take orally with or without food, as directed by a physician. Swallow the extended-release tablet whole; do not crush, chew, or break it.
Mechanism of Action
Rifaximin exerts its bactericidal effect by binding to the beta subunit of bacterial DNA-dependent RNA polymerase, thereby inhibiting bacterial RNA synthesis. This action is localized to the gut lumen due to its poor systemic absorption, effectively reducing the bacterial load in the gastrointestinal tract.
Pharmacokinetics
Onset
Symptomatic improvement for traveler's diarrhea typically within 24-48 hours. For IBS-D and HE, onset is gradual over days to weeks.
Excretion
Primarily excreted unchanged in feces (>96% of oral dose). Very small amount excreted in urine (<0.4%).
Half life
Approximately 2-3 hours (for the small amount systemically absorbed).
Absorption
Less than 0.4% systemic absorption; acts locally within the gastrointestinal tract.
Metabolism
Minimal hepatic metabolism by CYP3A4, with most metabolism occurring in the gut lumen.
Side Effects
Contraindications
- •Hypersensitivity to rifaximin, rifamycin antibiotics, or any component of the formulation
- •Patients with complicated traveler's diarrhea (fever, bloody stool, or severe symptoms)
- •Diarrhea caused by pathogens other than non-invasive E. coli
Drug Interactions
Warfarin
Rifaximin may alter gut flora, potentially affecting vitamin K production and warfarin efficacy; monitor INR closely.
P-glycoprotein inhibitors (e.g., Cyclosporine)
May increase systemic exposure to rifaximin, though clinical significance is generally low due to minimal absorption.
Storage
Store at room temperature (below 30°C/86°F), away from moisture and direct light. Keep out of reach of children.
Overdose
No specific antidote. Management of overdose should include symptomatic and supportive treatment as appropriate.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is unknown if rifaximin is excreted in human milk; caution is advised during breastfeeding.
