Description
Overview of the medicine
Renib is a selective Janus Kinase (JAK) inhibitor used for the treatment of various inflammatory and autoimmune conditions, such as rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis.
Uses & Indications
Dosage
Adults
40 mg once daily, with or without food.
Elderly
No dosage adjustment generally required, but monitor renal and hepatic function carefully.
Renal_impairment
Dose adjustment may be necessary in moderate to severe renal impairment. In severe impairment, consider 20 mg once daily.
How to Take
Take the tablet orally, with or without food. Do not chew, crush, or break the tablet.
Mechanism of Action
Renib inhibits the activity of Janus Kinase (JAK) enzymes (primarily JAK1 and JAK3), which are involved in the signaling pathways of various pro-inflammatory cytokines. By blocking these pathways, Renib modulates the immune response and reduces inflammation.
Pharmacokinetics
Onset
Clinical efficacy typically observed within 2-4 weeks of initiation.
Excretion
Approximately 76% excreted via urine and 20% via feces.
Half life
Approximately 4-6 hours.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations within 1-2 hours. Food does not significantly affect absorption.
Metabolism
Primarily metabolized by cytochrome P450 3A4 (CYP3A4) enzymes in the liver.
Side Effects
Contraindications
- Active serious infections
- Severe hepatic impairment
- Hypersensitivity to Renib or any excipients
- Active tuberculosis
Drug Interactions
Strong CYP3A4 inducers (e.g., Rifampicin)
May decrease Renib concentrations, potentially reducing efficacy.
Other immunosuppressants (e.g., Cyclosporine)
Increased risk of serious infections and other adverse reactions.
Strong CYP3A4 inhibitors (e.g., Ketoconazole)
May increase Renib concentrations, requiring dose reduction.
Storage
Store below 25°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for Renib overdose. In case of overdose, provide symptomatic and supportive care. Hemodialysis may be considered if necessary.
Pregnancy & Lactation
Renib is not recommended during pregnancy due to potential fetal harm. For lactating mothers, a decision must be made whether to discontinue breastfeeding or discontinue the drug, considering the potential risk to the infant.
