Description
Overview of the medicine
Renisol is a combination diuretic containing Amiloride Hydrochloride and Furosemide. It is used to treat high blood pressure (hypertension) and fluid retention (edema) caused by heart failure, kidney disease, or liver disease. Amiloride helps to prevent the loss of potassium that can occur with Furosemide.
Uses & Indications
Dosage
Adults
Typically, one tablet once daily, preferably in the morning. Dosage may be adjusted by a physician based on patient response.
Elderly
Lower initial doses may be required, with careful titration under medical supervision due to increased risk of electrolyte imbalance and renal impairment.
Renal_impairment
Contraindicated in severe renal impairment (anuria, acute renal failure). Use with extreme caution in moderate renal impairment, and dosage adjustment may be necessary. Serum electrolytes and renal function should be closely monitored.
How to Take
Take orally, with or after food, preferably in the morning to avoid nocturnal diuresis.
Mechanism of Action
Furosemide is a loop diuretic that inhibits the reabsorption of sodium and chloride in the ascending limb of the loop of Henle, leading to increased excretion of water, sodium, chloride, potassium, and hydrogen ions. Amiloride is a potassium-sparing diuretic that acts on the distal tubules and collecting ducts to inhibit sodium reabsorption and potassium excretion.

Excretion
Furosemide: Primarily renal excretion (unchanged). Amiloride: Excreted unchanged in urine and feces.

Furosemide: 1-2 hours. Amiloride: 6-9 hours.
Absorption
Furosemide: Rapidly absorbed (60-70% oral bioavailability). Amiloride: Incompletely absorbed (30-50% oral bioavailability).
Metabolism
Furosemide: Partially metabolized in the liver. Amiloride: Not extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to amiloride, furosemide, or sulfonamide derivatives
- •Anuria or severe renal failure
- •Severe hepatic impairment with hepatic coma or pre-coma
- •Pre-existing hyperkalemia (serum potassium > 5.5 mEq/L)
- •Concomitant use with other potassium-sparing agents or potassium supplements
Drug Interactions
NSAIDs
May reduce the diuretic and antihypertensive effects of furosemide, and increase the risk of renal impairment.
Digoxin
Furosemide-induced hypokalemia can potentiate digoxin toxicity.
Lithium
Diuretics can reduce renal clearance of lithium, leading to increased lithium levels and toxicity.
Aminoglycosides/Ototoxic drugs
Increased risk of ototoxicity with furosemide.
ACE inhibitors/Angiotensin II receptor blockers
Increased risk of hyperkalemia and severe hypotension.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe dehydration, electrolyte imbalances (e.g., hypokalemia, hyponatremia, hyperkalemia from amiloride), hypotension, dizziness, and gastrointestinal disturbances. Management involves supportive care, correction of fluid and electrolyte imbalances, and discontinuation of the drug.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus, as furosemide crosses the placental barrier. Furosemide and amiloride are excreted in breast milk; therefore, caution should be exercised when administering to a nursing mother. Consult a doctor.
