Description
Overview of the medicine
Reomen 50 mg Tablet is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure and heart failure. It works by relaxing blood vessels, thereby lowering blood pressure and improving blood flow.
Uses & Indications
Dosage
Adults
The usual starting dose is 50 mg once daily. The dose may be increased to 100 mg once daily if needed. Maximum daily dose is 100 mg.
Elderly
No initial dosage adjustment is typically required; however, a starting dose of 25 mg once daily may be considered for frail or elderly patients.
Renal_impairment
For patients with moderate to severe renal impairment (CrCl < 30 mL/min), a lower starting dose (e.g., 25 mg once daily) may be considered, with careful monitoring.
How to Take
Reomen 50 mg Tablet should be taken orally, with or without food, preferably at the same time each day.
Mechanism of Action
Reomen works by selectively blocking the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, reduced blood pressure, and decreased fluid retention.
Pharmacokinetics
Onset
Antihypertensive effect usually starts within 1-2 hours, with full effect observed within 3-6 weeks.
Excretion
Excreted primarily via feces (80-90%) and urine (10-20%) as unchanged drug and metabolites.
Half life
Elimination half-life is approximately 6-9 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 30-40%. Peak plasma concentrations reached within 1-2 hours.
Metabolism
Undergoes minimal hepatic metabolism via CYP enzymes. Primarily metabolized to an active carboxylic acid metabolite.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Pregnancy (especially in the second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m²)
Drug Interactions
Diuretics and other antihypertensive agents
May increase the hypotensive effect of Reomen.
Non-steroidal anti-inflammatory drugs (NSAIDs)
May reduce the antihypertensive effect of Reomen and increase the risk of renal impairment, especially in elderly or dehydrated patients.
Potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy Category D. Reomen is contraindicated in pregnancy, especially during the second and third trimesters, due to the risk of fetal harm. It is not known whether Reomen is excreted in human milk; caution should be exercised when administered to a nursing mother.
