Description
Overview of the medicine
Losartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure, diabetic nephropathy, and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.
Uses & Indications
Dosage
Adults
Initial dose typically 50 mg once daily. Can be increased to 100 mg once daily if needed.
Elderly
No initial dosage adjustment generally required unless severe renal impairment is present.
Renal_impairment
Initial dose 25 mg once daily in patients with severe renal impairment (creatinine clearance < 30 mL/min) or on hemodialysis.
How to Take
May be taken with or without food.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor, primarily found in vascular smooth muscle and the adrenal gland, leading to vasodilation, decreased aldosterone secretion, and reduced blood pressure.
Pharmacokinetics
Onset
Blood pressure reduction typically seen within 1 week, maximal effect in 3-6 weeks.
Excretion
Approximately 35% excreted in urine, 60% in feces, mainly as unchanged drug and active metabolite.
Half life
Losartan: 2 hours; Active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed orally, bioavailability about 33%. Peak plasma concentration reached in 1 hour for Losartan, 3-4 hours for active metabolite.
Metabolism
Extensively metabolized by CYP2C9 and CYP3A4 to an active carboxylic acid metabolite (E-3174) and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Losartan or any component of the formulation
- Pregnancy (especially 2nd and 3rd trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment (eGFR < 60 mL/min/1.73 m²)
Drug Interactions
Lithium
Increased serum lithium levels and toxicity.
NSAIDs (e.g., Ibuprofen)
May reduce antihypertensive effect and increase risk of renal impairment.
Diuretics (e.g., Furosemide)
Increased risk of hypotension.
Potassium-sparing diuretics (e.g., Spironolactone) / Potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypotension and tachycardia; bradycardia could occur. Management involves symptomatic and supportive treatment, including IV fluids for hypotension. Losartan and its active metabolite are not removed by hemodialysis.
Pregnancy & Lactation
Contraindicated in 2nd and 3rd trimesters of pregnancy due to fetal toxicity. Not recommended during breastfeeding.
