Description
Overview of the medicine
Repidor 100 mg Tablet contains Cefpodoxime Proxetil, a third-generation cephalosporin antibiotic used to treat various bacterial infections including respiratory tract, urinary tract, skin, and soft tissue infections.
Uses & Indications
Dosage
Adults
100-200 mg every 12 hours, depending on the severity and type of infection. Usually for 5-10 days.
Elderly
No specific dose adjustment is needed unless renal function is impaired. Dose based on creatinine clearance.
Renal_impairment
Dose adjustment required based on creatinine clearance (e.g., for CrCl < 30 mL/min, dose interval should be increased to 24 hours).
How to Take
Repidor 100 mg Tablet should be taken orally with food to enhance absorption. Do not crush or chew the tablet.
Mechanism of Action
Cefpodoxime Proxetil is a prodrug that is hydrolyzed to its active metabolite, cefpodoxime. Cefpodoxime acts by inhibiting bacterial cell wall synthesis, leading to bacterial cell death (bactericidal action).
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-3 hours.
Excretion
Primarily excreted unchanged in the urine (approximately 30-35% of the dose).
Half life
Approximately 2-3 hours.
Absorption
Approximately 50% absorbed from the gastrointestinal tract; absorption is increased when taken with food.
Metabolism
Prodrug is hydrolyzed to active cefpodoxime in the intestinal wall.
Side Effects
Contraindications
- Known hypersensitivity to cefpodoxime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reaction to any penicillin.
Drug Interactions
Probenecid
Increases plasma concentrations of cefpodoxime by decreasing its renal excretion.
Nephrotoxic drugs (e.g., aminoglycosides)
Concurrent administration may increase the risk of nephrotoxicity. Monitor renal function.
Antacids, H2-receptor antagonists, PPIs (e.g., omeprazole)
Can decrease the absorption of cefpodoxime, leading to reduced effectiveness. Administer cefpodoxime at least 2 hours before or after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be given. Gastric lavage may be considered. Hemodialysis or peritoneal dialysis may aid in removal of cefpodoxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Cefpodoxime should be used during pregnancy only if clearly needed. It is excreted in human milk; use with caution in nursing mothers.

YesClinical Trials
Cefpodoxime has undergone extensive clinical trials demonstrating its efficacy and safety in various bacterial infections. Ongoing post-marketing surveillance continues to monitor its long-term effects.
Lab Monitoring
- Monitor renal function (creatinine clearance) in patients with pre-existing renal impairment or during prolonged high-dose therapy.
- Periodic monitoring of complete blood count (CBC) and liver function tests (LFTs) may be considered during prolonged treatment.
Doctor Notes
- Assess patient's renal function before prescribing, especially in elderly or those with pre-existing kidney disease.
- Educate patients on the importance of completing the full course of therapy.
- Monitor for signs of *Clostridium difficile* infection, particularly if severe, persistent diarrhea occurs.
Patient Guidelines
- Complete the entire course of medication, even if symptoms improve.
- Take the tablet with food to reduce stomach upset and improve absorption.
- Do not save medication for future infections.
- Inform your doctor if diarrhea persists or becomes severe.
