Description
Overview of the medicine
Repopain 10 mg Tablet contains Tizanidine Hydrochloride, a centrally acting skeletal muscle relaxant used to manage spasticity associated with multiple sclerosis, spinal cord injury, or other neurological conditions. It works by blocking nerve impulses that are sent from the brain to the muscles, thereby helping to relax the muscles and relieve pain.
Uses & Indications
Dosage
Adults
Initial dose 2 mg orally once daily, gradually increased to an optimal dose, typically 2-4 mg three times daily. Max 36 mg/day.
Elderly
Start with a lower dose (e.g., 2 mg once daily) and titrate slowly due to potential for increased sensitivity.
Renal_impairment
Reduce dose in patients with creatinine clearance <25 mL/min; initial dose 2 mg once daily, titrate slowly.
How to Take
Repopain 10 mg tablet should be taken orally. It can be taken with or without food, but consistency in administration with respect to food is recommended. Do not crush, chew, or break the tablet.
Mechanism of Action
Tizanidine is an alpha-2 adrenergic agonist. It acts by increasing presynaptic inhibition of motor neurons at the alpha-2 adrenergic receptors in the spinal cord, leading to a reduction in muscle tone. This reduces spasticity and muscle spasms.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Mainly excreted by the kidneys (approximately 60% in urine as metabolites, 20% unchanged).
Half life
Approximately 2.5 hours
Absorption
Rapidly absorbed, with peak plasma concentrations occurring 1-2 hours post-dose. Bioavailability is about 40%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 1A2 (CYP1A2) isoenzyme.
Side Effects
Contraindications
- Hypersensitivity to Tizanidine or any component of the formulation
- Concomitant use with strong CYP1A2 inhibitors (e.g., fluvoxamine, ciprofloxacin)
- Severe hepatic impairment
Drug Interactions
Alcohol
May increase CNS depressant effects and hypotension.
Oral contraceptives
May increase tizanidine exposure by reducing its clearance.
Fluvoxamine, Ciprofloxacin
Significant increase in tizanidine levels, leading to severe hypotension and CNS depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, bradycardia, hypotension, and respiratory depression. Treatment is symptomatic and supportive, including gastric lavage and supportive care for vital functions.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefits outweigh the risks. Excretion into breast milk is unknown; caution is advised when administered to a nursing mother.
