Overview of the medicine
Risperidone is an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder, and irritability associated with autistic disorder. It helps to restore the balance of certain natural substances in the brain (neurotransmitters).
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg once daily, target dose 4-8 mg/day. Bipolar mania: Initially 2-3 mg once daily. Autistic irritability: Based on weight, often starting at 0.5 mg/day.
Elderly
Start with 0.5 mg twice daily. Increase dose slowly if needed.
Renal_impairment
Start with 0.5 mg twice daily. Max 1.5 mg twice daily. Adjust with caution.
How to Take
Take orally, with or without food, usually once or twice a day as directed by your doctor. Swallow the tablet whole with water.
Mechanism of Action
Risperidone works by blocking dopamine D2 receptors and serotonin 5-HT2A receptors in the brain. This action helps to reduce the positive and negative symptoms of schizophrenia and stabilize mood in bipolar disorder.

Onset
Initial effects may be seen within days, but full therapeutic effects typically take several weeks.
Excretion
Approximately 70% of the dose is excreted in the urine and 14% in the feces; primarily as metabolites.

Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations of risperidone are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to 9-hydroxyrisperidone (active and equipotent to risperidone). A smaller extent by CYP3A4.

Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management is supportive, including maintaining an open airway, oxygenation, ventilation, and monitoring of cardiovascular function.

Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted into breast milk; caution advised or consider discontinuing nursing.
