Overview of the medicine
Resitone (Piracetam) is a nootropic drug primarily used to improve cognitive functions, especially in conditions involving memory loss, attention deficits, and impaired mental performance. It is also used in cortical myoclonus and vertigo.
Uses & Indications
Dosage
Adults
For cognitive disorders: 2.4-4.8 g/day in 2-3 divided doses. For cortical myoclonus: Initially 7.2 g/day, gradually increasing by 4.8 g every 3-4 days up to a maximum of 24 g/day in 2-3 divided doses.
Elderly
Dosage adjustment is necessary based on renal function. Use with caution.
Renal_impairment
Reduce dose based on creatinine clearance: CrCl 50-79 ml/min: 2/3 of usual dose in 2-3 divided doses; CrCl 30-49 ml/min: 1/3 of usual dose in 2 divided doses; CrCl <30 ml/min: Not recommended. Not for severe renal failure.
How to Take
Tablets can be taken with or without food. Swallow whole with water. Do not chew or crush.
Mechanism of Action
Piracetam enhances cholinergic and glutamatergic neurotransmission, improves neuronal membrane fluidity, facilitates cerebral blood flow, and protects neurons from hypoxic damage. It influences neuronal and vascular functions without being a direct stimulant or sedative.

Onset
Effects may gradually become apparent over several weeks of consistent treatment.
Excretion
Primarily excreted unchanged in urine, with 80-100% of the dose eliminated within 24-48 hours.
Half life
Approximately 4-5 hours in plasma and 6-8 hours in cerebrospinal fluid.
Absorption
Rapid and almost complete absorption from the gastrointestinal tract. Peak plasma levels are reached within 0.5-1 hour.
Metabolism
Piracetam is not metabolized in the human body.
Side Effects
Contraindications
- Hypersensitivity to Piracetam or other pyrrolidone derivatives or to any excipients.
- Severe renal impairment (creatinine clearance < 20 ml/min).
- Cerebral hemorrhage.
- Huntington's chorea.
Drug Interactions
Thyroid hormones
Concurrent use with thyroid extracts may cause confusion, irritability, and sleep disturbances.
CNS depressants/stimulants
No significant interactions, but caution is advised with other CNS active agents.
Anticoagulants (e.g., Warfarin)
May potentiate the effects of anticoagulants and prolong prothrombin time.
Storage
Store in a cool and dry place, below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, symptomatic and supportive treatment should be administered. There is no specific antidote.
