Description
Overview of the medicine
Resopan (Pantoprazole) is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome, and to prevent NSAID-induced ulcers.
Uses & Indications
Dosage
Adults
Typical dosage for GERD and erosive esophagitis is 40 mg once daily for 4-8 weeks. For Zollinger-Ellison syndrome, dosage may vary and be higher.
Elderly
No dosage adjustment is generally required in elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dosage adjustment is generally required in patients with renal impairment.
How to Take
Take orally, usually once daily, before a meal. Do not crush or chew the tablet; swallow it whole.
Mechanism of Action
Pantoprazole works by irreversibly binding to the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid production.
Pharmacokinetics
Onset
Onset of action within 2.5 hours.
Excretion
Primarily excreted renally (approximately 80%) and the remainder in feces.
Half life
Terminal elimination half-life is about 1 hour.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to pantoprazole or any component of the formulation.
- Hypersensitivity to other substituted benzimidazoles.
Drug Interactions
Warfarin
Increased INR and prothrombin time observed in patients receiving PPIs and warfarin concomitantly.
Clopidogrel
Concomitant use may reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase and prolong serum levels of methotrexate, particularly at high doses.
Drugs dependent on gastric pH for absorption
Reduced absorption of drugs like ketoconazole, atazanavir, nelfinavir due to increased gastric pH.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.

There is limited experience with overdose. Symptoms may include mild gastrointestinal disturbances and drowsiness. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. It is excreted in human milk, so caution is advised during breastfeeding.
