Description
Overview of the medicine
A metered dose inhaler containing salbutamol (albuterol), used to relieve symptoms of asthma and COPD such as wheezing and shortness of breath.
Uses & Indications
Dosage
Adults
For relief of acute bronchospasm: 1-2 puffs (100-200 mcg) as needed. For prevention of exercise-induced bronchospasm: 2 puffs (200 mcg) 15-30 minutes before exercise. Maximum 8 puffs in 24 hours.
Elderly
Same as adult dosage, but caution with cardiovascular conditions. Start with lowest effective dose.
Renal_impairment
No specific dose adjustment required for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Exhale completely, then place the mouthpiece in your mouth. Press down on the canister while inhaling slowly and deeply. Hold your breath for about 10 seconds. Exhale slowly. If a second puff is needed, wait 30 seconds before repeating.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in the smooth muscle of the bronchial tree, leading to relaxation of the smooth muscles and bronchodilation.
Pharmacokinetics
Onset
Within 5-15 minutes
Excretion
Primarily via urine, as unchanged drug (40-80%) and metabolites (20%).
Half life
Approximately 4-6 hours (systemic)
Absorption
Rapidly absorbed from the lungs following inhalation. Systemic absorption is minimal but sufficient to cause systemic effects.
Metabolism
Partially metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any components of the inhaler
- Severe milk protein allergy (for some lactose-containing formulations)
Drug Interactions
Digoxin
May decrease serum digoxin levels.
Diuretics (e.g., Furosemide)
Increased risk of hypokalemia (low potassium levels) when used concurrently.
Beta-blockers (e.g., Propranolol)
Can block the bronchodilatory effect of salbutamol and may cause severe bronchospasm in asthmatic patients.
Tricyclic antidepressants and MAO inhibitors
May potentiate the cardiovascular effects of salbutamol.
Storage
Store below 30°C. Protect from frost and direct sunlight. Do not refrigerate or freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, palpitations, headache, dizziness, nervousness, and hyperglycemia. Severe hypokalemia can occur. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus (Pregnancy Category C). Caution is advised during lactation as salbutamol may be excreted in breast milk. Consult a doctor.

Lab Monitoring
- Monitor serum potassium levels, especially in patients with severe asthma, concurrent diuretic use, or conditions predisposing to hypokalemia.
Doctor Notes
- Emphasize proper inhaler technique to optimize drug delivery and minimize side effects.
- Advise patients against exceeding the prescribed dose due to increased risk of adverse cardiac events.
- Educate patients that increased use of rescue inhalers signifies worsening disease and requires re-evaluation of their treatment plan.
Patient Guidelines
- Read the patient information leaflet carefully before using the inhaler.
- Always follow your doctor's instructions on how and when to use your inhaler.
- Ensure proper inhaler technique as demonstrated by your doctor, nurse, or pharmacist.
- If you need to use your rescue inhaler more often than prescribed, contact your doctor immediately as this may indicate worsening asthma control.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one.
Driving Precautions
Generally, Respihaler does not affect the ability to drive or operate machinery. However, if you experience side effects like dizziness, tremors, or palpitations, avoid driving or operating machinery until symptoms subside.
Lifestyle Advice
- Identify and avoid triggers that worsen your asthma or COPD (e.g., smoke, pollen, dust mites).
- Maintain a healthy lifestyle, including regular exercise (as advised by your doctor) and a balanced diet.
- Quit smoking and avoid exposure to secondhand smoke.
