Overview of the medicine
Revoc-DS 250 mg Suspension is an oral antibiotic used to treat various bacterial infections. It contains Cefixime, a third-generation cephalosporin, effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Adults may take 400 mg daily. For suspension, dosage is usually based on weight for children.
Elderly
No dosage adjustment needed unless severe renal impairment.
Renal_impairment
Dosage adjustment required for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min: 75% of standard dose. For CrCl < 20 mL/min: 50% of standard dose.
How to Take
Shake the bottle well before each use. Can be taken with or without food. Complete the full course of medication as prescribed, even if symptoms improve.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Clinical improvement usually seen within 24-48 hours.
Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion. A small portion is excreted via bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed orally. Absorption is not significantly altered by food.
Metabolism
Not extensively metabolized in the body.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporins
- History of severe allergic reactions to penicillin (cross-reactivity is possible)
Drug Interactions
Warfarin
May increase prothrombin time (PT) and INR, increasing risk of bleeding. Close monitoring of coagulation parameters is recommended.
Probenecid
May increase cefixime blood levels by decreasing renal tubular secretion.
Oral contraceptives
May reduce the effectiveness of oral contraceptives, advise additional contraceptive methods.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days (check specific product leaflet).
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in low concentrations in breast milk, use with caution during lactation.
