Overview of the medicine
Revodil-625 mg Tablet is an antibiotic medicine used to treat a wide range of bacterial infections. It is a combination of Amoxicillin, a penicillin-type antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor.
Uses & Indications
Dosage
Adults
Typically, one 625 mg tablet every 8 to 12 hours, depending on the severity of the infection. Treatment duration is usually 5-7 days, or as directed by a physician.
Elderly
Similar to adult dosage, but dose adjustment may be necessary in elderly patients with impaired renal function.
Renal_impairment
Dosage adjustment is required based on creatinine clearance (CrCl). For CrCl >30 mL/min, no adjustment needed. For CrCl 10-30 mL/min, 625 mg every 12 hours. For CrCl <10 mL/min, 625 mg every 24 hours.
How to Take
Take orally, preferably at the start of a meal to minimize potential gastrointestinal intolerance. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis, leading to bacterial death. Clavulanic acid prevents the inactivation of amoxicillin by beta-lactamase enzymes produced by resistant bacteria.
Pharmacokinetics
Onset
Rapid, typically within 1 hour.
Excretion
Both components are primarily eliminated unchanged via renal excretion (glomerular filtration and tubular secretion) within 6 hours.
Half life
Amoxicillin: approximately 1 to 1.5 hours. Clavulanic acid: approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are achieved within approximately 1 hour.
Metabolism
Amoxicillin is partially metabolized to an inactive penicilloic acid. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to amoxicillin, clavulanic acid, or any other penicillin antibiotic.
- •History of cholestatic jaundice or hepatic dysfunction associated with previous amoxicillin/clavulanic acid therapy.
- •Severe renal impairment (CrCl <30 mL/min) without appropriate dose adjustment.
Drug Interactions
Probenecid
Concomitant use with probenecid increases amoxicillin plasma concentrations by decreasing renal tubular secretion.
Allopurinol
Increased incidence of rash when taken concurrently with amoxicillin.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance of methotrexate.
Oral contraceptives
Rarely, may reduce the efficacy of oral contraceptives; advise patients to use alternative non-hormonal contraception.
Oral anticoagulants (e.g., Warfarin)
May prolong prothrombin time; careful monitoring of INR is advised.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal symptoms and fluid/electrolyte imbalance. Gastric lavage and symptomatic and supportive treatment are recommended. Hemodialysis can remove both amoxicillin and clavulanic acid from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, use in pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk; monitor breastfed infants for diarrhea, candidiasis, and skin rash.
