Overview of the medicine
Rezor Max (Esomeprazole) is a proton pump inhibitor that works by decreasing the amount of acid produced in the stomach. It is used to treat conditions like heartburn, difficulty swallowing, and persistent cough.
Uses & Indications
Dosage
Adults
For most indications, 20-40 mg once daily. For erosive esophagitis, 20 or 40 mg once daily for 4 to 8 weeks. For H. pylori eradication, 20 mg twice daily for 7-14 days.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Rezor Max capsules should be swallowed whole with water at least one hour before a meal. Do not chew or crush the capsules.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that irreversibly binds to the H+/K+-ATPase enzyme system (the proton pump) found on the secretory surface of the parietal cells of the stomach. This inhibits the final step in acid production, reducing gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 80% of metabolites are excreted in the urine, and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours. Bioavailability is approximately 64% after a single 40 mg dose and increases to 89% with repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased methotrexate levels; monitor for toxicity.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antivirals.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include lethargy, drowsiness, blurred vision, tachycardia, nausea, vomiting, sweating, headache, and dry mouth. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B/C. Use only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Esomeprazole is excreted in human breast milk; use with caution during lactation.
