Description
Overview of the medicine
Rhinocort 100 mcg Nasal Spray contains budesonide, a corticosteroid, used to treat symptoms of seasonal and perennial allergic rhinitis. It works by reducing inflammation in the nasal passages.
Uses & Indications
Dosage
Adults
Typically 1-2 sprays (50-100 mcg) into each nostril once daily.
Elderly
No specific dosage adjustment required, use adult dose.
Renal_impairment
No dosage adjustment required.
How to Take
Shake well before use. Prime the pump before first use or after 24 hours of non-use. Spray into each nostril while blocking the other, avoiding spraying into the septum.
Mechanism of Action
Budesonide is a potent glucocorticoid with strong topical anti-inflammatory action. It reduces inflammation by inhibiting the release of inflammatory mediators and suppressing immune cell activity in the nasal mucosa.
Pharmacokinetics
Onset
Initial effects within hours, full effect within a few days.
Excretion
Mainly via urine and feces as metabolites.
Half life
Approximately 2-3 hours.
Absorption
Low systemic bioavailability (approx. 10%) due to extensive first-pass metabolism in the liver and limited absorption from the nasal mucosa.
Metabolism
Primarily hepatic via CYP3A4 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to budesonide or any component of the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Cimetidine
May slightly increase budesonide levels, but usually not clinically significant.
Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May significantly increase systemic exposure to budesonide, potentially leading to systemic corticosteroid effects. Concomitant use should be avoided or monitored closely.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children. Protect from light.
Overdose
Acute overdose is unlikely to cause severe effects due to low systemic absorption. Chronic overdose may lead to systemic corticosteroid effects (e.g., Cushing's syndrome, adrenal suppression). Management is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use with caution during pregnancy if benefits outweigh risks. Budesonide is considered compatible with breastfeeding due to low systemic exposure.

Doctor Notes
- Counsel patients on proper administration technique to ensure optimal efficacy and minimize local side effects.
- Advise patients that full therapeutic effect may take several days, and adherence is crucial.
