Description
Overview of the medicine
Ribavirin is an antiviral medication used in combination with other antiviral agents (e.g., peginterferon alfa, direct-acting antivirals) for the treatment of chronic hepatitis C virus (HCV) infection. It is also used in certain other viral infections.
Uses & Indications
Dosage
Adults
For chronic HCV, usually 1000 mg/day or 1200 mg/day divided into two doses (e.g., <75 kg: 1000 mg/day; ≥75 kg: 1200 mg/day), in combination therapy. This means 5-6 Ribacee 200 mg capsules per day.
Elderly
No specific dose adjustment generally needed based on age alone, but renal function should be monitored.
Renal_impairment
Significant dose reduction required. Contraindicated if creatinine clearance (CrCl) < 50 mL/min.
How to Take
Oral, usually with food. Should be taken twice daily, morning and evening.
Mechanism of Action
Ribavirin is a guanosine analog that interferes with viral RNA synthesis and inhibits viral replication. It is phosphorylated intracellularly to active tri- and monophosphate derivatives. The triphosphate derivative inhibits viral RNA polymerase, and also acts as a competitive inhibitor of inosine monophosphate dehydrogenase, thereby depleting intracellular guanosine triphosphate (GTP) pools.

Not directly applicable for acute effects; therapeutic effects for HCV are long-term.

Primarily renal (61% excreted unchanged in urine), with a minor fecal elimination (12%).
Half life
Long and variable, terminal half-life is approximately 120-200 hours due to extensive distribution and slow elimination.
Absorption
Rapidly absorbed after oral administration. Bioavailability is variable (45-65%). Food increases bioavailability.
Metabolism
Metabolized by phosphorylation to active forms and by deribosylation and amide hydrolysis to a triazole carboxylic acid metabolite.

- Autoimmune hepatitis
Drug Interactions
Interferon alfa
Enhanced risk of side effects, especially neuropsychiatric and hematologic effects.
Didanosine (ddI)
Increased risk of pancreatitis, lactic acidosis, and peripheral neuropathy.
Zidovudine / Stavudine
Reduced efficacy of HIV antivirals; avoid co-administration if possible.
Azathioprine / Mercaptopurine
Increased risk of myelosuppression due to inhibition of inosine monophosphate dehydrogenase.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote. Treatment is symptomatic and supportive. Hemodialysis is not effective in removing ribavirin.
Pregnancy & Lactation
Pregnancy (Category X): Absolutely contraindicated. Use effective contraception during treatment and for 6 months after for both male and female patients. Two reliable forms of contraception are recommended. Lactation: Contraindicated. Breastfeeding should be discontinued during treatment.
