Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by a physician. Best taken with meals for better absorption.
Elderly
No specific dose adjustment is usually required for elderly patients, but renal function should be monitored, and hypercalcemia risk considered.
Renal_impairment
Use with caution in patients with renal impairment. Serum calcium and phosphate levels should be monitored regularly. Not recommended in severe renal impairment without medical supervision.
How to Take
Administer orally, preferably with meals to enhance calcium absorption and minimize gastrointestinal upset. The tablet can be swallowed whole with water.

Excretion
Calcium is primarily excreted via feces, with a smaller portion excreted renally. Vitamin D3 and its metabolites are mainly excreted in the bile and feces, with some renal excretion.
Half life
The half-life of Vitamin D3 (cholecalciferol) itself is about 24 hours, but its active metabolite, calcitriol, has a half-life of 3-6 hours. Calcium has a complex kinetic profile with various compartments.
Absorption
Calcium Carbonate absorption is approximately 20-30% of the ingested dose, enhanced by Vitamin D3 and acidic environment. Vitamin D3 is well absorbed from the small intestine, primarily in the presence of bile salts.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxycholecalciferol (calcifediol) and then in the kidney to 1,25-dihydroxycholecalciferol (calcitriol), the active form.

Drug Interactions
Bisphosphonates

Cardiac glycosides
Hypercalcemia can potentiate the effects of cardiac glycosides, leading to arrhythmias.
Thiazide diuretics
May reduce urinary calcium excretion, increasing the risk of hypercalcemia.
Tetracyclines & Quinolones
Calcium can chelate these antibiotics, reducing their absorption. Administer calcium at least 2-3 hours before or 4-6 hours after these antibiotics.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose primarily relate to hypercalcemia, including nausea, vomiting, constipation, abdominal pain, muscle weakness, fatigue, polydipsia, polyuria, bone pain, confusion, and in severe cases, cardiac arrhythmias and coma. Treatment involves discontinuation of the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Calcium and Vitamin D are essential during pregnancy and lactation. This supplement can be used if medically indicated to meet increased requirements, but dosage should be determined by a healthcare professional.
