Description
Overview of the medicine
Rifacol 200 mg Tablet contains Rifaximin, a non-systemic antibiotic used to treat bacterial infections in the gut, such as traveler's diarrhea caused by non-invasive E. coli. It is minimally absorbed into the bloodstream, acting primarily in the gastrointestinal tract.
Uses & Indications
Dosage
Adults
For Traveler's Diarrhea: 200 mg three times daily for 3 days. For other indications like HE or IBS-D, higher strengths (e.g., 550 mg) are typically used.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No dose adjustment is considered necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
Take Rifacol 200 mg Tablet orally with or without food, as directed by your doctor. Swallow the tablet whole with a glass of water.
Mechanism of Action
Rifaximin inhibits bacterial RNA synthesis by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase. Its low systemic absorption allows it to act locally in the gastrointestinal tract, reducing bacterial overgrowth and toxin production.
Pharmacokinetics
Onset
Onset of action varies by condition; typically within 24-48 hours for traveler's diarrhea.
Excretion
Over 96% of the administered dose is excreted unchanged in the feces, reflecting its low systemic absorption.
Half life
Plasma half-life is short, approximately 5-6 hours for the minimal systemic component. Fecal excretion is the primary elimination route.
Absorption
Minimally absorbed systemically (<0.4%) after oral administration, leading to high concentrations in the gut lumen.
Metabolism
Undergoes limited metabolism, primarily by CYP3A4 enzymes in the liver for the small absorbed fraction.
Side Effects
Contraindications
- Hypersensitivity to rifaximin, rifamycins, or any component of the formulation.
- Diarrhea complicated by fever and/or bloody stools.
Drug Interactions
Warfarin
Limited data suggest minimal or no effect on INR in patients taking warfarin; however, monitoring of INR is advisable, especially at initiation or discontinuation.
Cyclosporine
Co-administration with potent P-glycoprotein (P-gp) inhibitors like cyclosporine may increase systemic exposure to rifaximin. Clinical significance is generally low due to minimal absorption.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, contact a healthcare professional or poison control center immediately. Treatment should be supportive and symptomatic, as rifaximin is minimally absorbed.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether rifaximin is excreted in human milk; use with caution during breastfeeding. Consult your doctor.

Lab Monitoring
- Routine lab monitoring is generally not required due to minimal systemic absorption. For hepatic encephalopathy, liver function tests and ammonia levels may be monitored.
Doctor Notes
- Ensure diarrhea is not complicated by fever or bloody stools before prescribing for traveler's diarrhea.
- Counsel patients on proper hygiene and complete course adherence.
- Consider 550 mg strength for HE recurrence prevention or IBS-D treatment.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if symptoms improve.
- Do not use if diarrhea is accompanied by fever or blood in the stool; consult your doctor immediately.
