Description
Overview of the medicine
Rifaximin 200 mg tablet is an oral antibiotic used to treat traveler's diarrhea caused by non-invasive strains of E. coli in adults and pediatric patients 12 years of age and older.
Uses & Indications
Dosage
Adults
For Traveler's Diarrhea: 200 mg orally three times daily for 3 days.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment due to minimal systemic absorption.
How to Take
Take orally with or without food.
Mechanism of Action
Rifaximin inhibits bacterial RNA synthesis by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase. Its action is primarily local within the gastrointestinal lumen due to minimal systemic absorption.
Pharmacokinetics
Onset
Symptomatic improvement for Traveler's Diarrhea within 24-48 hours.
Excretion
Over 96% of the administered dose is excreted unchanged in the feces. Less than 0.02% is excreted in urine.
Half life
Plasma half-life is approximately 5-6 hours, but most drug action is localized in the gut.
Absorption
Less than 0.4% systemically absorbed after oral administration, leading to very low plasma concentrations.
Metabolism
Minimally metabolized; primarily by CYP450 enzymes to a few minor metabolites.
Side Effects
Contraindications
- Hypersensitivity to rifaximin, any rifamycin antimicrobial agent, or any component of the formulation.
- Patients with diarrhea complicated by fever, bloody stools, or severe abdominal pain (suggesting invasive bacterial infection).
Drug Interactions
Warfarin
Although systemic absorption is minimal, monitor INR for patients on warfarin, as changes in gut flora can affect vitamin K synthesis.
P-glycoprotein inhibitors (e.g., Cyclosporine)
May significantly increase systemic exposure of rifaximin. Caution advised.
Storage
Store below 30°C (86°F). Protect from moisture and light. Keep in the original package. Do not freeze.
Overdose
Experience with rifaximin overdose is limited. In cases of overdose, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if rifaximin is excreted in human milk; caution should be exercised when administered to a nursing woman.
