Description
Overview of the medicine
Rinzin 10 mg Tablet contains Cetirizine Dihydrochloride, a second-generation antihistamine used to relieve symptoms of seasonal and perennial allergic rhinitis, and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
10 mg once daily. Some patients may respond to 5 mg once daily. Maximum daily dose is 10 mg.
Elderly
5 mg once daily initially, may be increased to 10 mg if well tolerated. Dose adjustment may be necessary due to reduced renal function.
Renal_impairment
Moderate renal impairment (creatinine clearance 30-49 mL/min): 5 mg once daily. Severe renal impairment (creatinine clearance < 30 mL/min): 5 mg every other day. Not recommended for end-stage renal disease patients on dialysis.
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Cetirizine is a potent and selective peripheral H1-receptor antagonist. It inhibits the early phase of histamine-induced allergic reactions and reduces the migration of inflammatory cells and the release of mediators associated with the late-phase allergic response.
Pharmacokinetics
Onset
Within 20-60 minutes.
Excretion
Primarily excreted in the urine (approximately 60% unchanged) within 24 hours. Small amount excreted in feces.
Half life
Approximately 8-11 hours in adults, longer in elderly and renally impaired patients.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1 hour after oral administration.
Metabolism
Undergoes limited hepatic metabolism; a significant portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives.
- Patients with severe renal impairment (creatinine clearance <10 mL/min).
- Patients undergoing hemodialysis.
Drug Interactions
Theophylline
A minor decrease in cetirizine clearance has been observed with concomitant theophylline use, but usually no dosage adjustment is needed.
Central Nervous System (CNS) Depressants
Concurrent use with alcohol or other CNS depressants (e.g., sedatives, tranquilizers) may increase CNS depression and impair alertness.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, tremor, headache, confusion, malaise, diarrhea, and urinary retention. In case of overdose, symptomatic or supportive treatment is recommended. Gastric lavage may be considered soon after ingestion. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cetirizine is excreted into breast milk; therefore, it is not recommended during breastfeeding unless the potential benefits outweigh the risks.

YesClinical Trials
Cetirizine has undergone extensive clinical trials demonstrating its efficacy and safety for treating allergic rhinitis and chronic idiopathic urticaria across various patient populations, including pediatric and elderly patients.
Lab Monitoring
- No routine lab monitoring is generally required. Renal function tests may be considered in patients with pre-existing renal impairment or elderly patients.
- Liver function tests in case of suspected hepatic impairment.
Doctor Notes
- Advise patients on potential sedation, especially when co-administered with other CNS depressants or alcohol.
- Dose adjustment is crucial for patients with renal impairment to avoid accumulation and increased side effects.
- Explain that while effective for symptom relief, it does not cure the underlying allergy.
Patient Guidelines
- Do not exceed the recommended dose.
- Avoid driving or operating heavy machinery if you experience drowsiness.
- Inform your doctor or pharmacist about all other medications you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cetirizine may cause drowsiness or dizziness in some individuals. Patients should be warned against engaging in activities requiring complete mental alertness, such as driving a car or operating hazardous machinery, until they are aware of their response to the drug.
Lifestyle Advice
- Identify and avoid triggers for your allergies, if possible.
- Maintain good indoor air quality to reduce exposure to allergens.
