Description
Overview of the medicine
RiPril 5 mg Tablet contains Ramipril, an Angiotensin-Converting Enzyme (ACE) inhibitor. It is used to treat high blood pressure, heart failure, and to reduce the risk of heart attack or stroke in patients at high risk.
Uses & Indications
Dosage
Adults
Hypertension: Initial 2.5 mg once daily, adjusted to 2.5-10 mg daily. Heart Failure: Initial 1.25 mg once daily, adjusted to a maximum of 10 mg daily. Post-MI: Initial 2.5 mg twice daily. Diabetic Nephropathy: Initial 1.25 mg once daily, adjusted to 2.5-10 mg daily.
Elderly
Lower initial doses may be required, titrate carefully.
Renal_impairment
Initial dose of 1.25 mg once daily, maximum of 5 mg daily for severe impairment.
How to Take
Take orally, with or without food, usually once daily at the same time each day. Swallow the tablet whole with a glass of water.
Mechanism of Action
Ramipril inhibits the Angiotensin-Converting Enzyme (ACE), preventing the conversion of angiotensin I to angiotensin II. This leads to decreased vasoconstriction, reduced aldosterone secretion, and lower blood pressure. It also reduces cardiac preload and afterload.

Onset
Starts acting within 1-2 hours, peak effect in 3-6 hours.
Excretion
Primarily renal (approximately 60%) and fecal (approximately 40%) excretion.
Half life
Approximately 13-17 hours for ramiprilat (active metabolite).
Absorption
Well absorbed after oral administration, approximately 50-60% bioavailable.
Metabolism
Extensively metabolized in the liver to ramiprilat (active) and inactive metabolites.
Side Effects
Contraindications
- History of angioedema related to previous ACE inhibitor treatment
- Hereditary or idiopathic angioedema
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment
- Bilateral renal artery stenosis
Drug Interactions
Lithium
Increased serum lithium levels and toxicity.
Aliskiren
Contraindicated in diabetic or renally impaired patients (increased risk of hyperkalemia, hypotension, and renal impairment).
Diuretics
Increased hypotensive effect.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce antihypertensive effect and increase risk of renal impairment.
Potassium-sparing diuretics, potassium supplements
Increased risk of hyperkalemia.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe hypotension, shock, bradycardia, electrolyte disturbances, and renal failure. Treatment is symptomatic and supportive, including intravenous fluid administration and monitoring of vital signs.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. Not recommended during breastfeeding as it may pass into breast milk.
