Overview of the medicine
Rivadem 3 mg capsule contains Rivastigmine, used for the treatment of mild to moderately severe dementia of the Alzheimer's type and mild to moderately severe dementia associated with Parkinson's disease.
Uses & Indications
Dosage
Adults
Initial dose is 1.5 mg twice daily. The dose should be gradually titrated up to the usual effective dose of 3-6 mg twice daily, with a maximum of 6 mg twice daily. Titration should occur at intervals of at least two weeks.
Elderly
No specific dose adjustment is required, but titration should be done carefully, considering the potential for increased sensitivity.
Renal_impairment
No specific dose adjustment is needed for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally with food, usually in the morning and evening. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Rivastigmine is a carbamate derivative that inhibits acetylcholinesterase and butyrylcholinesterase, thereby increasing the concentration of acetylcholine in the brain, which is deficient in dementia.

Onset
Cognitive effects may be observed within a few weeks of treatment, but full therapeutic benefits may take several months.
Excretion
Primarily renal, with over 90% of the dose excreted in urine within 24 hours. Less than 1% is excreted unchanged.
Half life
Approximately 1.5 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in approximately 1 hour. Bioavailability is about 36%.
Metabolism
Hydrolyzed primarily by cholinesterases, predominantly via the carboxyl esterase. Minimal involvement of cytochrome P450 isoenzymes.

Beta-blockers
May increase the risk of bradycardia and syncope when co-administered.
Succinylcholine
