Overview of the medicine
Rizelium 5 mg Capsule contains Rizatriptan, which is used for the acute treatment of migraine attacks with or without aura. It helps to relieve migraine headache, pain, nausea, and sensitivity to light and sound.
Uses & Indications
Dosage
Adults
Recommended initial dose is 5 mg or 10 mg at the onset of migraine headache. If symptoms recur, a second dose may be taken after at least 2 hours. Maximum daily dose is 30 mg.
Elderly
No specific dose adjustment required for elderly patients (over 65 years) without severe renal or hepatic impairment.
Renal_impairment
Caution advised in patients with severe renal impairment. A lower initial dose (e.g., 5 mg) may be considered.
How to Take
Take one capsule orally with water, as soon as migraine symptoms begin. Do not take more than 30 mg in a 24-hour period.
Mechanism of Action
Rizatriptan is a selective serotonin 5-HT1B/1D receptor agonist. It works by constricting dilated intracranial blood vessels and inhibiting the release of pro-inflammatory neuropeptides, thereby aborting migraine attacks.

Excretion
Mainly via the kidneys (approx. 80%) and feces (approx. 17%).

Approximately 2-3 hours.
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations are reached in 1-1.5 hours.

Primarily metabolized by monoamine oxidase-A (MAO-A) to an inactive indole acetic acid metabolite.

Contraindications
- Ischemic heart disease or vasospastic coronary artery disease
- History of stroke or transient ischemic attack (TIA)
- Peripheral vascular disease
- Uncontrolled hypertension
- Severe hepatic impairment
- Concomitant use with MAO inhibitors or within 2 weeks of discontinuing MAO inhibitors
- Concomitant use with other 5-HT1 agonists (e.g., sumatriptan, zolmitriptan) or ergot-type medications (e.g., ergotamine, dihydroergotamine).
Drug Interactions
Propranolol
Increases rizatriptan plasma concentrations; a lower rizatriptan dose (e.g., 5 mg) is recommended if co-administered.
SSRIs/SNRIs
Potential for serotonin syndrome; caution advised.
MAO inhibitors
Increased plasma levels of rizatriptan; contraindicated.
Other 5-HT1B/1D agonists
Increased risk of vasospastic reactions; contraindicated within 24 hours.

Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Caution advised during lactation; it is unknown if rizatriptan is excreted in human milk.
