Description
Overview of the medicine
Rmax 20 mg Capsule contains Omeprazole, a proton pump inhibitor (PPI) that reduces gastric acid secretion. It is used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, and stomach ulcers.
Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, depending on the condition. For GERD, 20 mg once daily for 4-8 weeks. For duodenal ulcer, 20 mg once daily for 2-4 weeks. For gastric ulcer, 20 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome, dosage is individualized, typically 60 mg once daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take Rmax 20 mg Capsule orally, preferably in the morning before breakfast. The capsule should be swallowed whole with water and not chewed, crushed, or opened.
Mechanism of Action
Omeprazole irreversibly blocks the hydrogen-potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This reduces both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour, with maximum effect within 2 hours. Full acid suppression achieved after 2-4 days of continuous dosing.
Excretion
Approximately 77% of a dose is excreted renally, and about 20% is excreted in the feces.
Half life
Plasma elimination half-life is short (0.5-1 hour), but the duration of antisecretory effect is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration, bioavailability is approximately 30-40% due to first-pass metabolism. Absorption is acid-labile; therefore, it is formulated as an enteric-coated capsule.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme system, specifically CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Omeprazole or any components of the formulation, or to other substituted benzimidazoles (e.g., esomeprazole, lansoprazole, pantoprazole, rabeprazole).
- Concomitant use with nelfinavir (an antiretroviral).
Drug Interactions
Diazepam
Decreased metabolism of diazepam, leading to prolonged elimination.
Warfarin
Increased INR and prothrombin time; monitor closely.
Tacrolimus
Increased serum concentration of tacrolimus.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased and prolonged elevation of methotrexate and its metabolite serum levels.
Nelfinavir, Atazanavir
Reduced plasma concentrations of antiretrovirals, potentially leading to loss of virologic response and development of resistance.

Overdose
Symptoms of overdose include drowsiness, headache, nausea, vomiting, tachycardia, and dry mouth. Treatment is symptomatic and supportive; there is no specific antidote. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
