Description
Overview of the medicine
Robust 20 mg Tablet is a novel antidepressant medication designed to treat major depressive disorder and various anxiety disorders by modulating serotonin levels in the brain, thereby improving mood and overall mental well-being.
Uses & Indications
Dosage
Adults
Initial dose 10 mg orally once daily. May be increased to 20 mg once daily after 1-2 weeks, based on clinical response and tolerability. Maximum recommended dose is 40 mg once daily.
Elderly
Initiate with a lower dose of 5 mg or 10 mg orally once daily. Dose titration should be gradual and cautious, considering potential decreased renal or hepatic function and increased sensitivity.
Renal_impairment
No dose adjustment is typically required for mild to moderate renal impairment. For severe renal impairment (CrCl < 30 mL/min), a lower starting dose (e.g., 10 mg once daily) and careful monitoring are recommended.
How to Take
Robust 20 mg Tablet should be taken orally once daily, with or without food. It is advisable to take the medication at the same time each day to maintain consistent drug levels.
Mechanism of Action
Robust 20 mg Tablet selectively inhibits the reuptake of serotonin in the central nervous system, leading to an increased concentration of serotonin in the synaptic cleft. This enhanced serotonergic neurotransmission is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial clinical effects may be observed within 1-2 weeks, but full therapeutic benefits typically require 2-4 weeks of continuous treatment.
Excretion
Excreted predominantly via the kidneys (approximately 60% as metabolites) and a lesser extent through feces (approximately 30%).
Half life
The elimination half-life is approximately 24-36 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with peak plasma concentrations reached within 4-6 hours post-administration. Bioavailability is approximately 80%.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP) enzymes, including CYP2D6 and CYP3A4, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to the active ingredient or any excipients of Robust 20 mg Tablet.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- Uncontrolled narrow-angle glaucoma.
- Severe hepatic impairment.
Drug Interactions
Triptans
Risk of Serotonin Syndrome.
Tricyclic Antidepressants (TCAs)
May increase plasma concentrations of TCAs, requiring dose adjustment.
Warfarin and other anticoagulants
Increased risk of bleeding due to potential inhibition of platelet aggregation.
Monoamine Oxidase Inhibitors (MAOIs)
Concomitant use may lead to serious, sometimes fatal, reactions including Serotonin Syndrome.
Non-steroidal Anti-inflammatory Drugs (NSAIDs)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, somnolence, dizziness, tremor, tachycardia, and rarely, seizures or serotonin syndrome. Management involves supportive and symptomatic treatment, including maintaining an open airway, cardiac monitoring, and administration of activated charcoal if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Robust 20 mg Tablet should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother. Consult your doctor.
