Description
Overview of the medicine
Rocal-D Vita 600 mg Tablet is a nutritional supplement containing elemental calcium and Vitamin D3 (Cholecalciferol). It is primarily used to prevent and treat calcium and Vitamin D deficiencies, which are crucial for bone health, muscle function, and overall well-being.
Uses & Indications
Dosage
Adults
Usually 1 tablet daily, or as directed by the physician. It is generally recommended to take with food.
Elderly
Same as adult dosage, but caution should be exercised in patients with renal impairment.
Renal_impairment
Use with caution and monitor calcium levels. Dosage adjustment may be required; consult a physician.
How to Take
Oral administration. The tablet can be taken with or without food, but generally recommended with meals to enhance calcium absorption and reduce gastric upset. Do not crush or chew if not specified as chewable.
Mechanism of Action
Calcium is an essential mineral for bone formation and maintenance, nerve function, muscle contraction, and blood clotting. Vitamin D3 helps the body absorb calcium from the gastrointestinal tract and regulates calcium and phosphate levels, which are vital for bone mineralization. Together, they promote strong bones and prevent conditions like osteoporosis.
Pharmacokinetics
Onset
Therapeutic effects on bone mineral density develop over weeks to months.
Excretion
Calcium is primarily excreted via feces, with some renal excretion. Vitamin D3 metabolites are excreted mainly in bile and feces, with some renal excretion.
Half life
Calcium: Varies. Cholecalciferol: Approximately 24 hours for the parent compound, with active metabolites having longer half-lives (e.g., 25-hydroxyvitamin D has a half-life of 15-30 days).
Absorption
Calcium carbonate is absorbed in the small intestine, requiring an acidic environment. Cholecalciferol (Vitamin D3) is fat-soluble and absorbed in the small intestine, requiring bile salts.
Metabolism
Calcium is not metabolized. Cholecalciferol is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications
- •Hypersensitivity to calcium carbonate, cholecalciferol, or any component of the formulation
- •Hypercalcemia (high calcium levels in blood)
- •Severe hypercalciuria (high calcium levels in urine)
- •Nephrolithiasis (kidney stones)
- •Hypervitaminosis D (excess Vitamin D in the body)
Drug Interactions
Bisphosphonates
Calcium can interfere with absorption. Administer at least 30-60 minutes apart.
Corticosteroids
May reduce calcium absorption.
Thiazide diuretics
May increase risk of hypercalcemia.
Digitalis glycosides
Hypercalcemia may increase the risk of digitalis toxicity.
Tetracyclines and Fluoroquinolones
Calcium can chelate these antibiotics, reducing their absorption. Administer at least 2-4 hours apart.
Thyroid hormones (e.g., Levothyroxine)
Calcium can impair absorption of thyroid hormones. Administer at least 4 hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia and hypervitaminosis D) include nausea, vomiting, constipation, thirst, polyuria, anorexia, weakness, and fatigue. In severe cases, it can lead to renal damage and cardiac arrhythmias. Treatment involves discontinuing the supplement, hydrating the patient, and managing symptoms under medical supervision.
Pregnancy & Lactation
Considered safe and often recommended during pregnancy and lactation for increased calcium and Vitamin D requirements, but should be used under medical supervision to avoid exceeding recommended daily allowances.
