Description
Overview of the medicine
Ronac Plus is a combination medicine containing Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID), and Paracetamol (Acetaminophen), an analgesic and antipyretic. It is used to relieve pain and inflammation associated with various conditions.
Uses & Indications
Dosage
Adults
One tablet 2-3 times daily, preferably after food.
Elderly
Lower doses may be required; monitor for adverse effects.

How to Take
Oral administration. Take with a glass of water, preferably after meals to minimize gastrointestinal upset.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, reducing inflammation, pain, and fever. Paracetamol primarily inhibits prostaglandin synthesis in the central nervous system, producing analgesic and antipyretic effects.

Excretion
Mainly via urine (Diclofenac as metabolites, Paracetamol as conjugates and metabolites).
Half life
Diclofenac: 1-2 hours; Paracetamol: 1.5-3 hours.
Absorption
Both Diclofenac and Paracetamol are well absorbed orally. Diclofenac peak plasma levels are reached in 2-3 hours; Paracetamol in 30-60 minutes.
Metabolism
Both undergo hepatic metabolism (Diclofenac via CYP2C9, UGT; Paracetamol via glucuronidation and sulfation).

Contraindications
- Hypersensitivity to Diclofenac, Paracetamol, or other NSAIDs
- Active gastrointestinal ulceration, bleeding, or perforation
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with paracetamol.
Warfarin/Anticoagulants
Increased risk of bleeding.
Diuretics/ACE inhibitors
Reduced antihypertensive effect; increased risk of renal impairment.
Lithium/Digoxin/Methotrexate
Increased plasma levels.

Increased risk of GI adverse effects.

Symptoms may include nausea, vomiting, abdominal pain, drowsiness, and GI bleeding. Paracetamol overdose can lead to severe hepatotoxicity. Diclofenac overdose can cause acute renal failure. Treatment is supportive; N-acetylcysteine for paracetamol overdose. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of ductus arteriosus. Use with caution in first and second trimesters only if potential benefit outweighs risk. Diclofenac and Paracetamol pass into breast milk; generally considered safe in usual doses during lactation, but caution is advised.
