Uses & Indications
Dosage
Adults
1 gm administered intravenously every 8 hours. Dosage may vary based on infection severity and type.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function. Renal function should be monitored.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 26-50 mL/min: 1 gm every 12 hours. For CrCl 10-25 mL/min: 500 mg every 12 hours. For CrCl <10 mL/min: 500 mg every 24 hours.
How to Take
Ropenem 1 gm injection is for intravenous use only. It should be reconstituted with an appropriate diluent (e.g., Water for Injection, 0.9% Sodium Chloride) and administered as an intravenous infusion over 15-30 minutes, or as a bolus over 3-5 minutes for certain infections.
Mechanism of Action
Ropenem exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) inside the bacterial cell, which interferes with the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Drug Interactions
Probenecid

Concurrent use may decrease valproic acid levels, leading to loss of seizure control. Co-administration is generally not recommended.
Oral anticoagulants
May enhance the anticoagulant effect of warfarin and other oral anticoagulants.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, discontinue Ropenem and institute general supportive treatment. Ropenem can be removed by hemodialysis.

Pregnancy Category B. Use in pregnancy only if clearly needed. Ropenem is excreted in human milk; caution should be exercised when administered to a nursing woman.
