Description
Overview of the medicine
Rosicard 5 mg Tablet contains Rosuvastatin, a statin medication used to lower high cholesterol and triglycerides in the blood, thereby reducing the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose 5 mg or 10 mg once daily. For patients requiring less aggressive LDL-C reduction, 5 mg once daily may be considered. Dose adjustment should be made at 2-4 week intervals. Maximum dose is 40 mg once daily.
Elderly
For patients >70 years, a starting dose of 5 mg is recommended.
Renal_impairment
In severe renal impairment (CrCl <30 mL/min) not on hemodialysis, the starting dose should be 5 mg and should not exceed 10 mg once daily. No dosage adjustment for mild to moderate renal impairment.
How to Take
Take orally once daily, with or without food. Can be taken at any time of day.
Mechanism of Action
Rosuvastatin is a selective, competitive inhibitor of HMG-CoA reductase, the enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. It reduces LDL-C, Total-C, ApoB, and TG, and increases HDL-C.

Onset
Lipid-lowering effect is evident within 1 week of therapy, with 90% of maximum response achieved by 2 weeks.
Excretion
Excreted primarily in feces (approximately 90%), with minimal renal excretion.
Half life
Approximately 19 hours.
Absorption
Approximately 20% absolute bioavailability. Peak plasma concentrations are reached in 3 to 5 hours.
Metabolism
Limited metabolism, primarily by CYP2C9 (approximately 10%).
Side Effects
Contraindications
- Hypersensitivity to rosuvastatin or any component of the formulation
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Severe renal impairment (creatinine clearance <30 mL/min)
- Myopathy
- Concomitant cyclosporine use
- Pregnancy and lactation
Drug Interactions
Ezetimibe
Increased risk of muscle problems.
Gemfibrozil
Increases rosuvastatin exposure; avoid co-administration if possible.
Cyclosporine
Significant increase in rosuvastatin exposure; co-administration contraindicated.
Erythromycin
Decreased rosuvastatin exposure.
Warfarin/Coumarin anticoagulants
Increased INR; monitor INR closely.
Protease inhibitors (e.g., Lopinavir/Ritonavir)
Increased rosuvastatin exposure; dose adjustment may be necessary.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

There is no specific treatment for rosuvastatin overdose. In the event of an overdose, symptomatic and supportive measures should be instituted. Hemodialysis is unlikely to be of benefit.
