Description
Overview of the medicine
Rosugen 10 mg Tablet is used to lower high cholesterol levels and reduce the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose 5-10 mg orally once daily. Maximum dose 40 mg once daily.
Elderly
Starting dose of 5 mg is recommended for patients ≥70 years, but generally same as adults, with caution for higher doses.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min), initial dose of 5 mg once daily, not to exceed 10 mg once daily.
How to Take
Take orally once daily, with or without food, at any time of day, but preferably at the same time each day.
Mechanism of Action
Rosuvastatin inhibits HMG-CoA reductase, an enzyme responsible for cholesterol synthesis in the liver, thereby reducing LDL ('bad') cholesterol and increasing HDL ('good') cholesterol.
Pharmacokinetics
Onset
Lipid-lowering effects are evident within 1 week, with maximal effect generally achieved by 2-4 weeks of therapy.
Excretion
Approximately 90% of rosuvastatin is excreted unchanged in the feces, and a small portion in the urine.
Half life
The elimination half-life of rosuvastatin is approximately 19 hours.
Absorption
Peak plasma concentrations are achieved in approximately 3-5 hours. Absolute bioavailability is about 20%.
Metabolism
Rosuvastatin undergoes limited metabolism, primarily by cytochrome P450 isoenzyme CYP2C9.
Side Effects
Contraindications
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy
- Lactation
- Hypersensitivity to rosuvastatin or any component of the formulation
- Severe renal impairment (CrCl <30 mL/min)
Drug Interactions
Warfarin
May enhance the anticoagulant effect. INR monitoring required.
Gemfibrozil
May increase rosuvastatin concentrations. Avoid concomitant use or limit rosuvastatin dose to 10 mg daily.
Cyclosporine
Coadministration significantly increases rosuvastatin exposure (up to 7-fold). Avoid combination.
Erythromycin
May decrease rosuvastatin exposure. Clinical significance is usually minimal.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide symptomatic treatment and supportive measures. Hemodialysis is unlikely to be of benefit.
Pregnancy & Lactation
Contraindicated in pregnancy (Category X) and lactation due to potential harm to the fetus/infant. Women of childbearing potential should use adequate contraception.
