Overview of the medicine
Rosulip 10 mg Tablet contains Rosuvastatin, which is an HMG-CoA reductase inhibitor used to lower high cholesterol and triglyceride levels, and to reduce the risk of cardiovascular diseases like heart attack and stroke.
Uses & Indications
Dosage
Adults
Initial dose 5-10 mg orally once daily. Maximum recommended dose is 40 mg once daily.
Elderly
For patients >70 years, a starting dose of 5 mg once daily is recommended. No other dose adjustments are generally needed.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min) not on hemodialysis, start with 5 mg once daily, and do not exceed 10 mg once daily.
How to Take
Take Rosulip 10 mg Tablet orally once daily, with or without food. It can be taken at any time of the day, but it is often recommended to take statins in the evening.
Mechanism of Action
Rosuvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This results in decreased hepatic cholesterol synthesis, leading to increased LDL receptor expression on hepatocyte surfaces, which enhances uptake and catabolism of LDL-C.
Pharmacokinetics
Onset
Lipid-modifying effects are usually evident within 1 week of therapy, with maximal effect by 2-4 weeks.
Excretion
Primarily excreted in feces (90%), with a minor proportion eliminated via urine.
Half life
Approximately 19 hours.
Absorption
Approximately 20% absolute bioavailability, peak plasma concentrations achieved in 3-5 hours.
Metabolism
Limited metabolism; primarily by CYP2C9. N-desmethyl rosuvastatin is the major metabolite.
Side Effects
Contraindications
- •Hypersensitivity to rosuvastatin or any component of the formulation.
- •Active liver disease, including unexplained persistent elevations of serum transaminases.
- •Pregnancy and lactation.
- •Concomitant use of cyclosporine.
Drug Interactions
Colchicine
Increased risk of myopathy.
Cyclosporine
Concomitant use is contraindicated due to significantly increased rosuvastatin exposure.
Gemfibrozil, Fenofibrate
Increased risk of myopathy/rhabdomyolysis.
Niacin (lipid-lowering doses)
Increased risk of myopathy/rhabdomyolysis.
Warfarin/other anticoagulants
May potentiate anticoagulant effect, increasing INR. Monitor INR closely.
Protease Inhibitors (e.g., ritonavir-boosted lopinavir)
Significantly increases rosuvastatin exposure; dose adjustment required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for rosuvastatin overdose. In case of an overdose, symptomatic and supportive measures should be taken. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
Rosulip is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use effective contraception during treatment.
