Overview of the medicine
Roxagen (Roxithromycin) is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain genitourinary infections.
Uses & Indications
Dosage
Adults
150 mg twice daily or 300 mg once daily, preferably before meals. Duration usually 5-10 days, depending on infection.
Elderly
No specific dose adjustment is generally required, but caution advised in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is usually required as only a small amount is excreted renally.
How to Take
Take Roxagen tablets orally, preferably at least 15 minutes before meals to ensure optimal absorption.
Mechanism of Action
Roxithromycin inhibits bacterial protein synthesis by reversibly binding to the 50S ribosomal subunit of susceptible microorganisms, thereby blocking transpeptidation.

Onset
Peak plasma concentrations are typically achieved within 1-2 hours after oral administration.
Excretion
Mainly excreted in faeces (approximately 65%), with a small amount (about 7-12%) excreted in urine.
Half life
Approximately 10-12 hours.
Absorption
Well absorbed orally, approximately 50% bioavailability. Food may delay absorption.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to roxithromycin, other macrolides, or any component of the formulation.
- Concomitant administration with ergot alkaloids (e.g., ergotamine, dihydroergotamine) due to potential for severe vasoconstriction.
- Concomitant administration with cisapride, pimozide, or terfenadine due to risk of QT prolongation and cardiac arrhythmias.
Drug Interactions
Digoxin
Increased digoxin levels, monitor digoxin concentrations.
Warfarin
Increased anticoagulant effect, monitor INR.
Midazolam
Increased sedative effect of midazolam.
Cyclosporine
Increased cyclosporine levels, monitor renal function and cyclosporine levels.
Ergot alkaloids
Severe peripheral vasoconstriction and ischemia (contraindicated).
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage and symptomatic supportive measures should be taken. There is no specific antidote for roxithromycin.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use in pregnancy only if clearly needed. Roxithromycin is excreted in breast milk; therefore, use with caution during lactation.
