Description
Overview of the medicine
Naproxen is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, fever, swelling, and stiffness. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
For pain/dysmenorrhea: 500 mg initially, then 250 mg every 6-8 hours as needed. Max 1250 mg/day for short term. For chronic conditions: 250-500 mg twice daily.
Elderly
Start with lower doses, closely monitor for adverse effects.
Renal_impairment
Caution advised, reduce dosage, monitor renal function. Avoid in severe renal impairment.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow whole with water. Do not crush or chew.
Mechanism of Action
Inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever.

Pain relief within 1 hour.
Excretion
Excreted mainly in the urine (about 95%) as unchanged naproxen, 6-O-desmethylnaproxen, and their conjugates.
Half life
12-17 hours.
Absorption
Rapidly and completely absorbed from the GI tract. Peak plasma concentrations are achieved in 2-4 hours.
Metabolism
Primarily metabolized in the liver by demethylation and subsequent conjugation.

Contraindications
- Hypersensitivity to naproxen or other NSAIDs
- History of asthma, urticaria, or allergic reactions after taking aspirin or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
Drug Interactions
Lithium
Increased lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.
ACE inhibitors/Diuretics
Reduced antihypertensive effect, increased risk of renal impairment.

Symptoms may include drowsiness, heartburn, indigestion, nausea, vomiting. Severe overdose may cause gastrointestinal bleeding, acute renal failure, hypertension, respiratory depression, and coma. Pregnancy & Lactation
Avoid in the third trimester due to risk of premature closure of fetal ductus arteriosus. Use with caution in first and second trimesters only if clearly needed. Excreted in breast milk, so caution advised during lactation.
