Description
Overview of the medicine
Roxinat 750 mg Injection contains Cefuroxime, a second-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and some sexually transmitted diseases. It works by killing susceptible bacteria.
Uses & Indications
Dosage
Adults
Typically 750 mg every 8 hours, administered intramuscularly or intravenously. For severe infections, 1.5 g every 8 hours.
Elderly
Dosage adjustment may be required based on renal function. Generally, adult doses are appropriate if renal function is normal.
Renal_impairment
Dosage frequency or amount should be reduced based on creatinine clearance. For CrCl 10-20 mL/min, 750 mg once daily; for CrCl <10 mL/min, 750 mg every 24 hours or longer interval.
How to Take
Roxinat 750 mg Injection is for intramuscular (IM) or intravenous (IV) administration only. It should be reconstituted with the appropriate solvent (e.g., Water for Injection) as per manufacturer's instructions and administered by a healthcare professional.
Mechanism of Action
Cefuroxime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, leading to inhibition of cell wall formation and subsequent bacterial cell lysis.
Pharmacokinetics
Onset
Rapid, usually within minutes for IV administration.
Excretion
Excreted unchanged primarily by glomerular filtration and tubular secretion via the kidneys. About 50% to 100% of a dose is eliminated within 24 hours depending on the route of administration.
Half life
Approximately 1-1.5 hours in healthy adults.
Absorption
Rapidly absorbed after intramuscular (IM) injection and peak plasma concentrations are achieved within 30-60 minutes. Intravenous (IV) administration provides immediate peak levels.
Metabolism
Cefuroxime is not metabolized in the body.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Probenecid reduces renal tubular secretion of cefuroxime, resulting in higher and more prolonged plasma concentrations.
Aminoglycosides
Concurrent use with aminoglycosides may increase the risk of nephrotoxicity.
Oral Anticoagulants
Cefuroxime may affect gut flora, leading to lower vitamin K synthesis and potentiation of anticoagulant effects (e.g., warfarin).
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze. Keep out of sight and reach of children.
Overdose
Overdose of cefuroxime can lead to neurological sequelae including convulsions. Management should be symptomatic and supportive. Hemodialysis and peritoneal dialysis can effectively remove cefuroxime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy should be considered only if clearly needed. Cefuroxime is excreted in breast milk in small amounts; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Renal function tests (especially in patients with pre-existing renal impairment or on prolonged therapy)
- Complete Blood Count (CBC) for prolonged use
- Liver function tests (if clinically indicated)
Doctor Notes
- Perform a thorough allergy history before administration, especially for beta-lactam hypersensitivity.
- Adjust dosage in patients with renal impairment based on creatinine clearance.
- Administer intravenously or deep intramuscularly as prescribed. Do not administer intra-arterially.
Patient Guidelines
- Complete the entire course of medication as prescribed by your doctor, even if your symptoms improve.
- Report any signs of an allergic reaction (e.g., rash, swelling, difficulty breathing) to your doctor immediately.
- Do not save unused medication for future infections, and do not share it with others.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose of Roxinat Injection is missed, contact your doctor or nurse immediately to reschedule. It is important to complete the full course of treatment as prescribed by your doctor, even if you start feeling better.
