Overview of the medicine
Rosuvastatin is an HMG-CoA reductase inhibitor (statin) used to lower high cholesterol and triglyceride levels, and to reduce the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose 5-10 mg once daily. Max dose 40 mg. The 10 mg tablet is a common starting dose.
Elderly
No specific dose adjustment generally needed, but caution with higher starting doses. Individualize dosage.
Renal_impairment
Mild-moderate: no adjustment. Severe (CrCl <30 mL/min): initiate 5 mg, max 10 mg daily.
How to Take
Take orally once daily, with or without food. Can be taken at any time of day, but consistency is recommended.
Mechanism of Action
Rosuvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early rate-limiting step in cholesterol biosynthesis. This leads to a decrease in hepatic cholesterol synthesis and an increase in the number of LDL receptors on the surface of liver cells, enhancing the uptake and catabolism of LDL.

Onset
Lipid-lowering effects typically within 1 week, maximal effect by 2-4 weeks.

Primarily feces (90%), some urine (10%).

Side Effects
Contraindications
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to rosuvastatin or any component of the formulation
- Severe renal impairment (CrCl <30 mL/min) not on dialysis, especially with concomitant cyclosporine
Drug Interactions
Gemfibrozil
Increases rosuvastatin exposure; avoid co-administration. If co-administration is necessary, limit rosuvastatin to 10 mg once daily.
Cyclosporine
Concomitant use increases rosuvastatin exposure significantly; limit rosuvastatin to 5 mg once daily.
Erythromycin
Decreases rosuvastatin exposure.
Other fibrates
Increased risk of myopathy.
Niacin (lipid-lowering doses)
Increased risk of myopathy.
Ritonavir-boosted lopinavir/atazanavir
Significantly increases rosuvastatin exposure; limit rosuvastatin dose.
Coumarin anticoagulants (e.g., Warfarin)
May increase INR; monitor INR closely.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide symptomatic treatment and supportive measures. Monitor liver function and CK levels.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation due to potential for fetal harm. Women of childbearing potential should use effective contraception.
